HuBERT Report Name: Mid‐Certification Assessment Report Topics: Frequency: CLN037 and OPR014 Mid‐Certification Assessments, Program Management On‐demand Access/Where to find: HuBERT Reports Environments: Clinic and State Office Description: This report displays a list of participants who have a mid‐certification assessment (MCA) due date within the specified date range. Totals are accumulated for each WIC type within each clinic, local agency, grantee, and for the state. For each participant, the following items are displayed: the Household ID, State WIC ID, name, date of birth, authorized representative’s name, phone number (Telephone 1), Last Date to Use (LDTU) of the last set of benefits issued, the certification effective and end dates, and the MCA due and completed dates. Note: A training module is available for staff performing MCAs, with report information at the end. Criteria and Filters: 
This report displays a list of participants who have a MCA due date within the specified date range. For most participants, the system calculates MCA due date as: o

Infants: Date of Birth + 9 months Children and Breastfeeding women: Cert Effective Date + 6 months Participants who have been terminated will display on this report (e.g., failed to pick up, over income).
A selection must be made for Clinic, Agency, Grantee, or State from the radio buttons. If “State” is chosen, the report will be quite lengthy, with all of the other levels included and the state totals at the end. The report is sorted in the following order: first by Grantee ID, then Agency ID, then Clinic ID, WIC Category and lastly by the participant’s Name. This report can be run for Infants, Children, and/or Breastfeeding participants. A MCA due date range is selected. The “From” date cannot be more than one year in the past. The “To” date cannot be more than one year in the future. Due to the 12‐hour delay when in the Reports Environment, this report only provides clinic activity up to and including the previous day, but not including the current day’s activities. All Mid‐Certifications The version provided by the State Office Reports Environment provides detail for each clinic, agency, grantee and WIC Type, with state totals on the last page. The title identifies the type(s) of MCA status included in the report.

This line subtotals the MCA types in this section of the report. Each page displays the Grantee, Agency, Clinic, and WIC Type selected. Spreadsheet: Total Due: count of those due on or after the current run date and in the date range. If the Missed or Completed report is selected, no Due records will display. Total Missed: count of those due before current run date and in the date range. If the Due or Completed report is selected, no Missed records will display. Total Completed: count of those due in the date range and with a MCA completed date. If the Due or Missed report is selected, no Completed records will display . The Excel spreadsheet option could be used to reformat the data. Page breaks and subtotals do not appear in the spreadsheet. WIC categories are displayed in a separate column.
