Program Management Reports HuBERT Report Name: Certification Due Reports/Letters Report Topics: Frequency: CLN002 Detail and Summary Program Management on demand Access/Where to find: HuBERT Reports Environment: Clinic Description: This report displays a list of participants who are enrolled at specified clinics and due for re-certification during the time frame selected. The detail report lists each member that meets the criteria, sorted by due date, then by last name, first name and middle initial. The household ID, state WIC ID, name, mailing address, phone number (Telephone 1), Termination Date and Termination Reason are displayed in the detail report. The summary reports displays the count for each clinic/agency by WIC Type. Criteria and Filters: • Participants must be due for certification in the timeframe selected. • Participants must belong to the selected clinic. • Participants who are eligible for recertification within the selected date range are included in the report, even if their current certification has been terminated. • The report does not include those participants that are becoming categorically ineligible, i.e., breastfeeding and non-breastfeeding women, and children turning five. • Agencies and clinics are suppressed if no participants meet the criteria for selection. Some possible purposes for this report: Caution! Due to the 12-hour delay when in the Reports Environment, this report only provides clinic activity up to and including the previous day, but not including the current day’s activities. • To get a list of participants who are due or overdue for recertification Output Options: • This report can be run by one or more clinics for the agency. • Summary Report, Detail Report, or Official Notices can be selected. Minnesota does not use this Official Notice entitled “Letter of Ineligibility.” • Select the Report to display on the screen, send directly to the chosen printer, save as a pdf, or save the raw data as spreadsheet. Caution! Printing “Official Notices” or labels sends an automated print job to the printer and prints letters for every participant. Once started, users are unable to cancel this print job! There will be a page break between clinics/agencies. If the participant has a certification due date within the selected dates and was terminated, the terminated date and reason will display. When you select dates, include the last day of the month, or the report will be blank for that month. Participants who are eligible to be recertified are counted in this report in the last WIC Category for which they were certified. The following are becoming categorically ineligible for one category, yet they may still eligible to be recertified as another WIC Type: • Pregnant women (P) are eligible to be recertified as postpartum (B or N) ; and • Infants (I) are eligible to be recertified as Children (C). Breastfeeding women (B) are certified in MN for one year and then become categorically ineligible. (This report is designed to be flexible enough to allow use by other states who certify breastfeeding women for a different timeframe (six months). “Non-Breastfeeding” women (N) are not eligible for recertification and N/A is displayed.
