Program Management Reports HuBERT Report Name: Benefit Pickup Report CLN006 Report Topics: Participation Access/Where to find: HuBERT Reports Environment: Clinic Description: This report displays a list of participants who have not come in for a nutrition visit and received benefits, as of a specified date. This report lists all members in the household that meet the criteria; the oldest member of the household is displayed first. The mailing address, phone number (Telephone 1), Household ID, State WIC ID, WIC Type, Cert End Date, Last Date to Use of the Last Set of Benefits, the Terminated Date (if applicable) and the Terminated Reason (if applicable) for each participant are also displayed. Criteria and Filters: • Participants must have a certification end date on or after the specified “As of Date.” • The “Last Date to Use of Last Set of Benefits” must be less than the date entered in the “As of Date” field. • The “As of Date” entered must be greater than or equal to today’s date. • Participants must be eligible for benefits. • Terminated participants will display on the report. The termination reason is provided for further evaluation. • Participants not due to receive food benefits (i.e., exclusively BF infants < 6 months old and some BF women > 6 months post partum) will not display. • If you want a list of “due” participants, enter a future date. Some possible purposes for this report: Caution! Due to the 12-hour delay when in the Reports Environment, this report only provides clinic activity up to and including the previous day, but not including the current day’s activities. • To identify participants who have not received benefits. • To get a list of households that should be picking up benefits. Output Options: • This report can be run by one or more clinics for the agency for which the user is logged on. • Labels: There is an option to generate Labels first. • Select the Report (default) or Letters to display on the screen, send directly to the chosen printer, save as a PDF, or Save Raw Data as Spreadsheet (not for Letters). o Caution! Display the report on the screen before printing, as it can be quite lengthy. Consider printing it to a pdf and printing only the pages needed. o Letter option: Prints one letter per household informing participants in the household that they can still receive benefits. Caution! Printing letters sends an automated print job to the printer and prints letters for every household. Once started, users are unable to cancel this print job! Clinic: If multiple clinics are selected, results for the next clinic will display for the same date range after the first clinic is completed. The participant’s phone number will print for the first household member listed on the report if it is available. Household ID: One household is indicated for each household ID. If blank for the participant, use the same Household ID as directly above the blank. TIP! Households near the bottom of the page may be split up—check for other household members on the next page of the report.
