Building Services Confidentiality Agreement
I understand that in the course of my work at Building Services I may have exposure to personal
medical information. I understand that this information, whatever its form or wherever it may appear,
is protected as private and confidential by law. As in all health care settings, personal health
information may only be accessed by persons directly involved in the care of the patient. It is the goal
of Health Services to maintain all printed medical records in locked storage after hours and to keep
the environment free of any other visible information about patients.
I understand that when I encounter personal medical information (including documents and reports of
any kind, names of patients, scheduling information, and the like) I will not discuss or share it with
anyone.
I understand that violation of this policy may be considered grounds for disciplinary action or
termination of my employment at Calvin College. I also understand that, if I have question, I may
discuss them with the Director of Health Services.
