Why Conduct a Community Health Assessment

available so that we could use the Community Health
Assessment as a tool to improve the health of our
community

Why Complete a Community Health Assessment
• It is what’s right for the community
– Communities differ from one to the next
– What is needed on one community may not be needed in
another
• Gives the community members an opportunity to give input on their
health issues
• Provides data to allow leaders to establish health priorities based on
identified community needs
• Helps promote healthy communities and healthy behaviors
• Assures quality and accessibility of health services

A community health assessment yields valuable market information and builds
relationships

A community assessment that provides a comprehensive picture of your
community’s health status and unmet needs—not only its health care
needs—can serve several strategic functions.
•
Assessments present an excellent opportunity to connect with your local
community and to strengthen partnerships with your public health
department, schools, YMCA, United Way, community agencies and other
hospitals. Collaborating with these mission-focused community
stakeholders to collect data and set health priorities can build trust and a
foundation for shared responsibility and commitment to tackle community
health needs.

The assessment process can be a venue to tell your story of community
service to influential advocates for health care.

For nonprofit hospitals, there is a new spotlight on community health
assessment with the release of the Internal Revenue Service’s updated
Form 990 and new Schedule H. In addition to asking hospitals to report on
their ―community health improvement services,‖ Schedule H specifically
inquires ―how the organization assesses the health care needs of the
communities it serves.‖
•
Information obtained from assessments can help target your community
health programs. Like all resources, community health and wellness dollars
need to be spent wisely. Understanding the prevalence of chronic health
conditions and barriers to access can help direct resources where they will
have the biggest impact.
•
Community health assessment can be a window to unmet needs, a guide to
targeting limited resources, a vehicle for strengthening important
relationships, and an invaluable tool for keeping your community healthy.

Our Community’s Health Assessment - 2010
• National Rural Health Resource Center (The Center) guided us
during the entire process
• Survey questions developed by Essentia Health- Northern Pines
Medical Center (Medical Center) and The Center
• The 6 zip codes in our service area were surveyed from a supplied
database with 800 residents being selected randomly
• 256 surveys returned
• 33% response rate
• 95% confidence level +/- 5.08%

Creating the Community Needs
Assessment Questions
• It was difficult to know what questions to ask.
• This was our first Community Needs Assessment and
we had no baseline data
• Trend data, demographics and other resources were
used in trying to create the best questions for our
community
• Some questions were based on questions that
community members had already asked such as will
there be additional outreach services after the affiliation
• The Center was instrumental in helping us create the
questions that would best serve us in trying to identify
opportunities and actions

Next Steps as a Result of Community
Health Assessment
• In order to increase access, we have hired a nurse
practitioner. who specializes in Geriatrics to work with
nursing home patients and another nurse practitioner
who will start later this summer
• Will be providing community education sessions

• Better prepared for the next Community Health
Assessment in three years
– Partner with our community leaders in creating
question
– Make it a community effort
