Complete “Individual Adjunctive Eligibility” exercise
Complete “Multiple Income Items” exercise
Complete “Copy Demographics” exercise
Complete “Link Mom & Baby” exercise
Complete “Linked Infant’s Health Information” exercise
Complete “Child Over 2 and Health Information” exercise
Complete “Height/Weight & Blood” exercise
Complete “Assign Risk Factors in Nutrition Assessment” exercise
Complete “Delete Nutrition Assessment contact” exercise
Household ID:
Complete a referral for infant - select an Immunizations referral.
Complete a referral for mom - select a Breastfeeding Resources referral
Complete a referral for the child – no referral needed.
Do not complete the rest of the CGS.
Go to: “Edit Referral Contact” exercise.
How to add a referral contact in the CGS:

In infant’s CGS, click on Referrals/Other Program link
o
Click Add button
o
Double click on Immunizations
o
Click Immunization organization to select
o
Click >> (Select) button
o
Click OK button
o
Click OK button
o
Click mom’s participant folder/CGS button on taskbar
o
In mom’s CGS, click on Referrals/Other Program link
o
Click Add button
o
Double click on Breastfeeding Resources
o
Click Breastfeeding Resource organization to select
o
Click >> (Select) button
o
Click OK button
o
Click OK button
o
Click child’s participant folder/CGS button on taskbar
o
In child’s CGS, click on Referrals/Other Program link

Click Add button
o
Double click on No Referral Needed
o
Click No Referral Needed organization to select
