CRITERIA:
Complete “Create New Household 2” exercise
Complete “Individual Adjunctive Eligibility” exercise
Complete “Multiple Income Items” exercise
Complete “Copy Demographics” exercise
Complete “Link Mom & Baby” exercise
Complete “Linked Infant’s Health Information” exercise
Complete “Child Over 2 and Health Information” exercise
Complete “Height/Weight & Blood” exercise

In this exercise, you are going to “accidentally” start the child’s Nutrition Assessment contact in the
infant’s folder.
Start a Nutrition Assessment contact in the infant’s CGS.
Answer first three questions in Nutrition Assessment for the child instead of the infant (as if accidentally
entering information into the wrong household member’s Nutrition Assessment contact).
Add risk factor 344 – Thyroid Disorders (child has thyroid condition, not infant).
Finish the Nutrition Assessment contact without answering any other questions.
Complete a Nutrition Assessment contact in the child’s CGS – answering as many questions as you
want.
Add risk factor 344 – Thyroid Disorders and an appropriate dietary risk factor (ex: 425 series).
Complete a Nutrition Assessment contact in the mom’s CGS.
Add an appropriate dietary risk factor (ex: 401).
Do not complete the rest of the CGS.
Go to: “Delete a Nutrition Assessment Contact” exercise.
How to assign Risk Factors in NUTRITION ASSESSMENT:

In infant’s CGS, click on Nutrition Assessment link
o
Click Add Contact button
o
Answer first three questions as if answering for child instead of infant

Type text into Answer field
•
Remember: If NONE is an appropriate answer, you do not have to type anything into
the Answer field
•
Click Next>> button to move to next Nutrition Assessment question
o
Click Add button to add risk factor
o
Click on 344 – Thyroid Disorders
o
Click OK button
o
Click Finish button to close Nutrition Assessment contact
o
Click Close button
o
Click child’s participant folder/CGS button on taskbar
o
Click on Nutrition Assessment link
o
Click Add Contact button
o
Answer some of the questions
o
Click the Add button to add risk factor
o
Click on 344 – Thyroid Disorders and a 425 risk factor
o
Click OK button
o
Answer a few more questions
o
Click Finish button to close Nutrition Assessment contact
o
Click Close button
o
Click mom’s participant folder/CGS button on taskbar
o
Repeat for mom except only select a dietary risk factor
