Complete “Individual Adjunctive Eligibility” exercise
Complete “Multiple Income Items” exercise
Complete “Copy Demographics” exercise
Complete “Link Mom & Baby” exercise
Complete “Linked Infant’s Health Information” exercise
Complete “Child Over 2 and Health Information” exercise
Household ID:
Complete Height/Weight and Blood for child; rev iew growth grids.
Referral data for hemoglobin from 45 days ago; hemoglobin v alue on referral data w as 8.2.
Enter referral data and enter hemoglobin from re-check today.
Complete Height/Weight and Blood for mom.
Complete Height/Weight for infant; rev iew growth grids.
Do not complete the rest of the CGS.
Go to: “Assign Risk Factors in Nutrition Assessment” exercise.
How to complete Height, Weight and Blood with referral information:
o In child’s CGS, click on Height/Weight and Blood link
o Click Add… button
o Enter Height Inches/Eighths
o Enter Weight Pounds/Ounces
o Click OK button
o Click Growth Grids… button to review
o Click different grid tabs to view all growth grids
o Click Close button
o Click Blood tab
o Click Add… button
o Enter Measurement Date of ~45 days prior to today

Tip: Must enter referral data before entering measurement for today. Cannot
enter values with measurement dates less than the most recent record.
o Enter 8.2 into Hemoglobin
o Click OK button
o Click Add… button
o Enter new hemoglobin value for today’s date
o
Click OK button
o Click Close button
How to complete Height/Weight and Blood for mom:
o Click button on taskbar to maximize mom’s CGS
o Click on Height/Weight and Blood link
o Click Add… button
o Enter Height Inches/Eighths
o Enter Weight Pounds/Ounces
o Click OK button
o Click Blood tab
o Click Add… button
o Enter Hemoglobin
o Click OK button
o Click Close button
How to complete infant’s Height/Weight:
o Click button on taskbar to maximize infant’s CGS
o Click on Height/Weight and Blood link
o Click Add… button
o Enter Height Inches/Eighths
o Enter Weight Pounds/Ounces
o Click OK button
o Click Growth Grids… button to review
o Click different grid tabs to view all growth grids
o Click Close button
