CRITERIA:
Complete “Create New Household 2” exercise
Complete “Individual Adjunctive Eligibility” exercise
Complete “Multiple Income Items” exercise
Complete “Copy Demographics” exercise
Complete “Link Mom & Baby” exercise
Household ID:
Baby is mostly-breastfeeding; baby started formula 2 days ago.
Complete Health I nformation for infant.
Refresh Health I nformation if information does not default from mom’s linked record.
Do not complete the rest of the CGS.
Go to: “Child Ov er 2 & Health I nformation” exercise
How to complete linked baby’s Health Information:
o
Click infant’s participant folder/CGS button on taskbar
o Click on Health Information link
o I f information does not default from mom’s record it is because system needs to refresh
infant’s information

Close infant’s CGS and participant folder and re-open

TIP: To av oid this issue, w ait until you’v e completed mom’s Demographics and
Health I nformation screens before opening baby’s CGS
o Enter Birth Facility- Do Not Collect
o Click OK button
