CRITERIA:
Complete “Create New Household 2” exercise
Complete “Individual Adjunctive Eligibility” exercise
Complete “Multiple Income Items” exercise
Complete “Copy Demographics” exercise
Household ID:
Link mom and baby from mom’s record.
Baby is mostly-breastfeeding; baby started formula 2 days ago.
Complete Health I nformation for mom.
Mom has Diabetes and had regular prenatal care during her pregnancy.
Verify mom receiv ed Risk Factor 343 – Diabetes Mellitus.
Do not complete the rest of the CGS.

How to link mom and baby in mom’s record:
o Click mom’s participant folder/CGS button on taskbar
o Click on Health Information link
o Enter a date in the expected delivery date field
o Enter infant’s Date of Birth in the Actual Delivery Date field

Can’t remember infant’s DOB? Simply click on clinic button on taskbar to
maximize participant list to see baby’s DOB, then re-minimize it
o Click Infant(s) Born from This Pregnancy button
o Click Add button
o Select Live at Postpartum Visit from Infant Status at Birth drop-down
o Click Infant On WIC checkbox
o Click Linked Participants icon (…)
o Scroll to find infant by State WIC ID or name and click to highlight
o Click OK button (a two-way link between mom and infant’s record is created)
o Complete rest of information

Gender (defaults from infant’s record)

Birth Height In/8ths

Birth W eight Lbs/Ozs

Ever Breastfed = Yes

Breastfeeding Now = selected

Amount of Breastfeeding = Mostly Breastfeeding

In actuality, you may not know the amount of breastfeeding based on
your discussion with mom so far. Select what seems correct and you can
adjust as necessary later.

Date Breastfeeding Verified = today

Date Breastfeeding Began = birthdate

Date Supplemental Feeding Began = 2 days ago
o Click OK button
o Click Close button

Enter Weight Gained during Pregnancy

Enter Weight at Delivery
o
Select DO NOT COLLECT from Birthing Facility drop-down
o
Click Diabetes Mellitus checkbox
o
Enter Date Prenatal Care Began (month/year)
o
Enter Cigarette Usage and Alcohol Intake
o
Click OK button
