CRITERIA:
Complete “Create New Household 2” exercise
Household ID:
Complete Demographics and I ncome for child in CGS.
Child is indiv idually adjunctiv e eligibility (SSI ).
Household Size is 4.
Do not complete the rest of the CGS.
Go to “Multiple I ncome I tems” exercise.
Child with individual adjunctive eligibility:
o Open child’s participant folder
o Click Certification icon
o Click Yes on “Do you want to start…?” message
o Click on Demographics link
o In Demographics in CGS, click on Income Calculator icon
o Click on SSI checkbox
o Select Eligibility Letter from proof drop-down
o Enter Household Size of 4
o Click OK button
o Complete Demographics for child











Household Smoking
TV/Video Viewing (if child over 2)
Medical Clinic
Marital Status = DO NOT COLLECT
Education Level = Unknown
Authorized Representative Name = mom’s name
