CRITERIA:
Pregnant w oman w ith cert end < today’s date or w ithin 30 days
Woman deliv ered a set of tw ins 1 w eek ago today. Woman is breastfeeding the tw ins.
• Tw in 1 is fully breastfeeding w ith no formula.
• Tw in 2 is mostly breastfeeding. Mom is giv ing 2 ounces of formula/day and needs 1
can of pow der Similac Adv ance.
Start CGS for all three family members. Complete required fields in Demographics, Health
I nformation, Ht/Wt/Blood, Nutrition Assessment, Risk Factors/High Risk, Referrals/Other
Programs.
Create appropriate food package for mom.
Create appropriate food package for tw ins.
How to create food packages for Mom and her breastfeeding twins:
o
Prescreen tw ins for this mother.
Begin CGS for mom:
o Certification WI C Category box
 Select Breastfeeding button
 On WI C during pregnancy?
• Yes
 Do you w ant to start a certification attempt for the participant?
• Yes
o Click Health Information and link the tw o infants.
 Tw in 1 = Fully breastfeeding.
 Tw in 2 = Mostly breastfeeding.
 Under Most Recent Pregnancy History – select Multi-fetal gestation. Risk
Factor 335 w ill be auto assigned by the system.
o Complete required fields for Demographics, Ht/Wt/Blood, Nutrition Assessment,
Risk Factors/High Risk, Referrals/Other Programs
o Click Food Prescription.
Begin CGS for tw in 1:
o Complete required fields for Demographics, Health I nformation, Ht/Wt/Blood,
Nutrition Assessment, Risk factors/High Risk, Referrals/Other Programs
o Click Food Prescription. Food Package w ill default in food package for fully
breastfed infant for first year.
Begin CGS for Tw in 2:
o Complete required fields for Demographics, Health I nformation, Ht/Wt/Blood,
Nutrition Assessment, Risk factors/High Risk, Referrals/Other Programs
o Click Food Prescription. Food Package w ill default in food package for Mostly
breastfed infant for first year.
 Edit quantity of formula to appropriate amounts for the first year.
