CRITERIA:
Prescreen a child
Parent’s income is $35000/year per taxes.
Household size is 3.
Mom is w illing to apply for MHCP-MN Care.
Complete income.
Complete Demographics.
Complete required documentation.
Complete the certification and issue benefits.
How to document presumptive eligibility:
o Click the Demographics link
o Click the Income Calculator icon
o Click the MHCP-MNCare checkbox
o Select Presumptive Eligibility – 90 days from the proof drop-down
o Click OK button
o Enter 3 into the Household Size field
o Click OK button
o Complete required Demographics fields
o Click OK button
o Complete the rest of the certification
o Issue 3 months of benefits
o Click Manage Notes icon
o Click the Create General Note button
o Select PE (Presumptive Eligibility) from the Subject of Note drop-down
o Document in note:



Household size (3)
Income amount ($35000/year)
Proof provided (tax form)
o Click OK button
o Click Close button
o Click Manage Alerts icon
o Click the Add button
o Enter into the alert text field:

Number of months benefits issued for and “Need documentation of pending
application or MHCP eligibility before more benefits issued” – or what you usually
write
o Click the OK button
o Click the Close button
