mom does not hav e proof of income.
I ncome = 28000/year.
Household size = 3.
Complete income.
Complete Demographics.
Complete the certification and issue benefits.
Complete required documentation.
How to document if proof of income is unavailable:
o Click the Demographics link
o Click the Income Calculator icon
o Click the Add item button
o Click the Yearly radio button
o Enter 28000 into Yearly Income field
o Select Waiver Signed from Proof of Income drop-dow n
o Click OK button
o Enter 3 into the Household Size field
o Click OK button
o Complete required Demographics fields
o Click OK button
o Complete the rest of the certification
o Print the Certification Notice
o Complete the No Proof of Income, Identity or Residence Waiver section
o Scan the document into the participant folder if you are attached to a scanner
