Out-of-State Transfer for
New BF Participant & Baby
CRITERIA:
BF mom has VOC document/I D folder – nev er been on MN WI C before – w as fully
breastfeeding and state she w as from only certified mom
Amount of Breastfeeding = Mostly-Breastfeeding
Baby new to WI C
Baby’s date of birth = today’s date minus 1 month (for example: today is 8/15/2012  DOB =
7/15/2012)
Perform out-of-state transfer for breastfeeding w oman nev er on MN WI C before.
Add baby to same household.
Baby’s First Name:
Baby’s Last Name:
Baby’s State WI C I D:
Link mom and baby’s records (use any height/w eight/w eeks gestation, etc.)
How to perform out-of-state transfer for breastfeeding woman never on MN WIC before and add
baby to household:
o Click Create New Household icon
o Enter Prescreen information
o Select Applicant Has VOC Document checkbox
o Enter Certification End Date

For breastfeeding woman = infant’s DOB + 1 year, last day of the month
o Select Add Another Member radio button
o Click OK button
o Complete VOC screen

Enter Actual Delivery Date

Select Mostly-Breastfeeding for Amount of Breastfeeding

Enter 1 for Priority
o Enter Prescreen information for infant
o Click OK button
o Open mom’s participant folder
o Click on Health Information tab
o Click Infant(s) Born from This Pregnancy
o Click Edit
o Click On WIC checkbox
o Click Link Participants icon (ellipses)
o Select infant from list
o Complete fields in screen
o Click OK button
