Local agency staff can run end-of month reports from the previous month from HuBERT’s Reports Environment usually between the 17th to the 20th of the current month. (Revised 4.11.16) Up through August 2014 this report was saved to the FileZilla (FTP) site and was named Fruit & Vegetables Redemption Activity. This report has one tab and provides both State and Local Agency information about fruit and vegetable cash-value benefits (CVBs). The output provides a count of issued, redeemed, and unused CVBs, along with the percentage redeemed, for women, children and infants for the specified participation month. Guidance Document Benefits & Redemption Count of Participants & Households Issued Benefits by Date & Time (REVISED 4.11.16) Revisions: This report used to be titled BENEFIT ISSUANCE BY DATE & TIME. It has been revised to provide only a count of issuance by participants and households. The participant detail lists have been removed since this information can also be found in the BENEFITS ISSUED ON DATE AT CLINIC and PARTICIPANTS ISSUED BENEFITS report templates. Also, the tabs have been combined so that there are only two tabs instead of six. This report has two tabs and provides a count of participants and households who were issued benefits during the specified time period by date and hour of the day Tab 1 provides a count based on all benefits issued. Tab 2 provides a count based only on nutrition visits (benefit pick-ups); participants who were issued benefits at a certification or Midcertification Assessment (MCA) on the same date as the benefits were issued aren’t included in the count. The report outputs are grouped by Clinic ID. (4.7.16) Guidance Document Benefits & Redemption Mailed Benefits (4.4.16) This report provides a list of participants who were mailed benefits issued during a specified time period by the specified agencies. The report output provides both the date and time the mailed benefits were issued along with the Staff ID of the person who issued them. Guidance Document Benefits & Redemption Mailed Benefits: Counted, Replaced & Redeemed (Revised – 4.4.16 – previously titled MAILED BENEFITS) Revisions: This report used to be titled MAILED BENEFITS. However, it provides more than a list of participants issued mailed benefits, which is what the new MAILED BENEFITS report (above) provides. This report indicates if the mailed benefits were counted for the participation month, if the mailed benefits were replaced, and whether they were redeemed. It can also only be run for one participation month and one agency at a time. This report has five tabs. The first tab provides a list and count of participants to whom benefits were mailed during the specified participation month by the specified agency. The Tab 2 provides a list and count of these participants whose mailed benefits were voided and whether they were counted as of the current date (see Notes). Tab 3 provides a subset of the participants whose mailed benefits were voided and who subsequently had a replacement set issued for that same month. Tab 4 provides redemption information for each mailed benefit for participants listed in Tab 1. The last tab provides total redemption for each participant listed in Tabs 1 and 4.
