Medical Formula Ordering Process
Now that Trend Fields has been hired to replace Theresa Graham, the State WIC staff person
who used to process your medical formula orders, please follow these steps to order a medical
formula beginning Monday, January 25th.
1. As you currently do, please complete a medical formula order form for each medical
formula you need to order, and make sure all items are filled out on the form including
any flavors of the product that may be preferred. The medical formula order form is

2. Please attach or scan the medical formula order form and send it by email to both
[email protected] and [email protected] (Under extenuating
circumstances, you could fax the form to Trend or Karen at 651/215-8951, but we would
prefer to receive all order forms by email.)
