Practice Transformation Work Group
Review Date: March 31, 2016
*Existing Work Groups are expected to review their charter once per year to ensure that it still accurately describes the
purpose and the structure of the team. The charter must be submitted to the Health Policy Division Director for approval,
even if no changes were made.
Purpose & Goals
The purpose of the Practice Transformation Work Group is to provide guidance and technical expertise to
the Health Care Homes program staff around ongoing practice transformation across the state. The
Practice Transformation Work Group shall:





Advise on development of practice transformation initiatives
Support implementation of practice transformation events
Advise on best practices and innovations regarding practice transformation
Advise on policies and procedures for practice transformation topics
Ensure patients, families, and consumers are included in Practice Transformation Work
Group activities
The goals of the Practice Transformation workgroup are to offer recommendations to:





Develop ongoing practice transformation and technical expertise to Health Care Homes
Staff
Examine HCH certification/recertification processes
Develop a diagram showing the alignment/linkages to HCH services payment models,
and initiatives such as BHH, MACRA, IHPs, ACOs, etc.
Improve population health and care coordination technical assistance to clinics.
Partner with MDH Office of Health Information Technology to provide technical
assistance to clinics to help them better understand e-health and health information
exchange.
*For the purposes of this document Health Care Homes (HCH) includes certified and potential HCH
providers and clinics.
Frequency of Meetings
The Practice Transformation Work Group will meet minimally once per quarter. Meetings will last

approximately two hours. Meetings will be held in-person with an option to participate via
webinar or conference call. Additional meetings will be scheduled if the Work Group feels
additional time is needed to get work done.
The meeting is considered public. Meeting notices will be posted on the MDH HCH website. In
addition, the meeting date and time will be posted on the Health Care Homes SIM Outlook
Calendar.
Membership Expectations
Practice Transformation Work Group members shall:
1.
Participate actively in meetings
2.
Provide input and direction on presented materials and questions
3.
Constructively represent the concerns and interests of their constituency
4.
Participate in ad hoc team initiatives as determined by the Work Group
5.
Champion the work of Health Care Homes and SIM in the community
Terms of Membership
Members are solicited by members and serve for two year terms. Membership is renewable for up to
three terms for a total of 6 years. Members of the will have staggered terms to ensure experienced Work
Group members are present on the group at all times.

Leadership
The Work Group will have two co-chairs:
 The co-chairs serve a two-year term from July 1 to June 30
 The co-chairs will be selected by the Work Group at the last meeting of the year
 The appointment times for each co-chair will be staggered
Co-Chair Responsibilities:
 Assist with developing meeting agendas
 Lead meetings and facilitate consensus decision-making
 Represent the at the HCH Advisory Committee or as needed
 Ensure communication with program staff

Decision Making Process
The Work Group makes decisions using a consensus process whenever possible; however, in order to move
forward, voting may be used at times. In the case of voting, at least half of the group must be present and
decisions will be made based on a 2/3 majority of those present. Decisions that involve activities requiring
mandatory staff attendance or activity costs that exceed five hundred dollars are forwarded to the Health
Care Homes Director.
Ad Hoc Teams
The Practice Transformation Work Group may establish ad hoc teams to complete short-term projects with
a specific assignment and a timeline.
Life Expectancy
This is an ongoing Work Group.
Reporting of Progress
This Work Group will report progress and recommendations to the HCH Advisory Committee. In turn the
HCH Advisory Committee will provide feedback to the Work Group. The Practice Transformation Work
Group will maintain final decision making authority.
Meeting agenda and materials available by request. The meeting will not be recorded.
