Tilden Middle School PTSA “Wish List” Request 2014-2015 The PTSA would like to use part of our funds to purchase items that benefit our students. All staff members are encouraged to make requests to the PTSA for “wish list” items. All requests will be evaluated by a committee led by parents and staff. You may want to talk to Ms. LaGrange before making your request to see if what you are asking for can be covered by school funds. To make a “wish list” request, please fill out the form below (for each item or class of items) and place the form in the PTSA President’s mailbox in the main office. Date: Name of person(s) requesting item: Department: What are you requesting? Please be as specific as you can, including total quantity requested. Is this a new item or an addition/upgrade to an existing item? Who will use this item (how many people)? (Which staff members and which students?) Will there be any training required to use the item? How much does the item cost? (Include all shipping, handling, installation, other costs).
