Returning expired MnVFC influenza vaccine

As you may know, injectable influenza vaccine will be expiring June 30. Follow the steps below to return
expired MnVFC influenza vaccine to McKesson. Do not return any vaccine, including influenza, prior to the
expiration date.
How to return expired MnVFC influenza vaccine
1) Remove expired influenza vaccine from the refrigerator or freezer. Mark it ‘nonviable’ to avoid using it
unintentionally.
2) Complete the Nonviable MDH Vaccine form stating the reason the influenza vaccine is nonviable (i.e.,
expired vaccine). You will find the form at: www.health.state.mn.us/vfc on the MnVFC Forms page.
3) Fax the Nonviable MDH Vaccine form to the MnVFC program at 651-201-5501.
4) Pack the influenza vaccine to protect it from breaking (refrigerated gel packs are not necessary) and
include the original Nonviable MDH Vaccine form in the package.
5) Keep a copy of the Nonviable MDH Vaccine form for your records.
MnVFC staff will coordinate a UPS pick-up within 2 weeks. Be sure to inform the front desk at your clinic that
UPS will be picking up the package in that time frame.
Please note: You will not receive a label ahead of time from McKesson, the UPS driver will arrive with a label.
Have more than one shipping package?
If you need to return more than one package of nonviable vaccine, please indicate that on the Nonviable MDH
Vaccine form. This will ensure you have the number of labels you need.
Do you have additional expired MnVFC vaccine?
Include information about all of your expired vaccine on the Nonviable MDH Vaccine form and you can return
all of your expired vaccine at the same time.
