Annual MnVFC Online Training due November 30, 2015
Your clinic’s immunization manager, vaccine coordinator, and their backups must complete MnVFC online training each
year to meet the federal VFC education requirement. This training highlights the MnVFC program requirements and
storage and handling best practices.
• The 2016 MnVFC Separate Stock Online Training is available at www.health.state.mn.us/vfc.
• The 2016 MnVFC Replacement Method Online Training is available at
www.health.state.mn.us/divs/idepc/immunize/mnvfc/replacement/index.html.
Provider Agreement, Annual Report, and Semi-Annual Report due November 30, 2015
• The Minnesota Vaccines for Children (MnVFC) Program Provider Agreement and the Annual Report of the Number of
Patients Immunized with Vaccine from MDH are available at www.health.state.mn.us/vfc.
• The Semi-Annual Report of Doses Administered to Pediatric Patients only needs to be completed by parent clinics
using the replacement method and is available at
www.health.state.mn.us/divs/idepc/immunize/mnvfc/replacement/index.html.

*Separate stock involves keeping privately-purchased vaccine separate from publicly-purchased vaccine. Patients must
receive vaccine from the correct vaccine “stock” based on their eligibility.
**The replacement method allows a site to use privately-purchased vaccine to vaccinate MnVFC-eligible children and
the MnVFC program replaces the private vaccine that was administered. Only sites with grandfathered status are
allowed to use the replacement method.
