Freshman only. Incomplete forms will not be processed.

I am a student athlete, and a course conflicts with my practice schedule. Please provide the sport you are involved in at TSU
below as well as your practice schedule. Please, also, indicate the course that conflicts with your practice schedule.
I am a member of the Aristocrat of Bands, and a course conflicts with my practice schedule. Please provide your practice
schedule and indicate the course that conflicts with your practice schedule.
I work, and a class conflicts with my schedule. Please provide your weekly work schedule below and indicate the course that
conflicts with your practice schedule.
I have Advanced Placement (AP) credit or dual enrollment credits for a course that is on my schedule.
Which course(s) have you received AP or dual enrollment credit? If AP, provide the test you took and the score.
I want to change my major and need the classes that fit with my new major. Please submit a completed change of major
form with this course change form.
I am an Honors student and need honors courses. Please list the courses that you need removed. Also list the honors
courses that you will be replacing the dropped courses with.

Your request will be reviewed by an academic advisor to determine if the submitted changes will be approved. You will be notified via the email address provided of whether or
not your change request has been approved. The changes made to your schedule may impact your financial aid and/or the ability to meet your scholarship standards. Please
note, schedule changes will be made based on course availability, learning support requirements, and degree plan requirements – we will not be able to maintain the same
course times as your current schedule. All changes will be final.
