CHANGE IN TITLE/ GUIDE NAME FOR M.PHARM (SEM IV) DISSERTATION (2013-2014)
This is to be inform to all the M. Pharm institute’s head that, in case of those M. Pharm (Sem IV)
students, for whom reviewers have suggested to do change in title of dissertation (During Mid sem
Review-2014)can send the request for the same in below mentioned format on Institute’s letter head duly
signed by Institute’s head and respective Guide of the Students. Also attach the scan copy of Review
Cards of those student’s along with cover letter.

The Last date of submission is 20 th March 2014 Thursday. After this date application will not be
entertained.
Note:
1. Title change will be allowed only on those cases. Wherein reviewers has put specific comment
for the same.
2. Send separate letter for title change in case of Students appeared in Remedial Exam (ITD/
Midsem Review).
3. In case of change in Guide name also submit scan copy of resignation letter of Previous Guide
and joining letter of New Guide.
