Cynthia Trevino B.S.W, L.S.W. 2016 Our Mission Open Door Health Center exists to increase access to quality, affordable, culturally appropriate, patient centered health care and to maximize the overall well-being of our patients. At Open Door Health Center, services are designed to meet the needs of low income and underserved communities facing language, cultural, and/or financial barriers to health care. History of a safety net 1980’s 1983 1993 2004 2009 2012 2013 2015 • One Physician occasionally sees patients at no cost • Became the Wellness Center • Paid Staff, Volunteers, and Clinic Space • Became Open Door Health Center • Provided Nurse Managed Care • Added Dental Services • Designated as a Federally Qualified Health Center • Added Behavioral Health Services • Launched Mobile Health Program to serve across Southern Minnesota • Awarded HRSA New Access Point Grant to increase services via Mobile Health • Awarded HRSA Capital Expansion Grant to renovate and expand our facility • Renovation of Holly Lane facility begins, completion expected in 2014-15. Clinic certified as a Medical Home program. We begin transition to an integrated behavioral health and community health approach to patient care. • New clinic in full operation. Significant progress is made toward integrated care. Service goals include the following: High quality, responsive, comprehensive, affordable, safe and accessible health care services. Culturally competent, community-oriented, and efficient care delivery. Coordinated health education programs and outreach services which promote health awareness, healthy lifestyles, disease prevention, and management of chronic illness. Individualized health care, appropriate to the needs of the patient. Effective advocacy for issues related to community health and community health centers.
