A. FAMILY PLANNING METHOD SERVICES
1. Unduplicated number of females receiving medical family planning method services where a client
chart exists by age, race, and ethnicity. This number should be equal to or greater than the number
listed under Section I. Improved Pregnancy Outcomes A.4. Medical family planning method services
means physical examination by a physician/nurse practitioner/certified nurse midwife and lab
services provided prior to the provision of a family planning method (regardless of the method
chosen). Services may be provided in public clinics or in private offices. Do not include in these
statistics services provided using funds received as a Family Planning Special Project grantee.

chart exists. Case management/care coordination refers to a set of activities provided by a health
professional to a child experiencing illness, disability, or impairment, or to their family to assure that

implementation of an individual plan of care, arrangement for services, monitoring of service
