Protecting, maintaining and improving the health of all Minnesotans January 5, 2015 Dear Title V Maternal and Child Health Block Grant Grantees: Re: Required acknowledgement of use of federal funds This letter is to inform you of the Federal Office of Fleet and Asset Management (OFAM) request that we ensure grantees are more fully acknowledging the Federal Health Resources and Services Administration (HRSA) as a funding source for their products/materials produced with federal grant funding. The required language is highlighted below. Please include the following language if you are using Title V Maternal and Child Health Block Grant as a funding source for a product or material: “This project is/was supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) under grant number B04MC28107 Maternal and Child Health Services (specify total award amount and percentage financed with nongovernmental sources). This information or content and conclusions are those of the author and should not be construed as the official position or policy of, nor should any endorsements be inferred by HRSA, HHS or the U.S. Government.” Grantees are required to use this language when issuing statements, press releases, requests for proposals, bid solicitations, and other HRSA supported publications and forums describing projects or programs funded in whole or in part with HRSA funding, including websites. Examples of HRSA-supported publications include, but are not limited to, manuals, toolkits, resource guides, case studies and issues briefs.
