National Nursing Home Quality Care Collaborative Details The Lake Superior Quality Innovation Network (LSQIN) offers your nursing home an opportunity to participate in the National Nursing Home Quality Care Collaborative. It will: • • • Help you work on quality and performance improvement practices Eliminate healthcare acquired conditions Dramatically improve resident satisfaction The collaborative complements other national and state initiatives, including Advancing Excellence in America’s Nursing Homes Campaign, The Partnership to Improve Dementia Care, Quality Assurance Performance Improvement (QAPI), the Minnesota DHS Performance-Based Incentive Payment Program (PIPP), and the Minnesota DHS Quality Improvement Incentive Payment Program (QIIP). The goal is to improve systems of care that lead to better quality outcomes for your nursing home residents. Targets will include increasing resident mobility, decreasing unnecessary use of antipsychotics in residents with dementia, decreasing potentially avoidable hospitalizations, and decreasing healthcare-acquired infections and conditions. The collaborative is free. As a participant, you will: • • • • Form an interdisciplinary team consisting of a minimum of four people, including senior leaders, direct care staff, and support service staff who provide resident care and services Participate in a minimum of six collaborative learning opportunities During periods between the collaborative events, you will test and implement the changes you choose and collect data to measure the impact of the changes Submit requested data/progress reports to Stratis Health, a partner in LSQIN The collaborative is a mutual commitment between the LSQIN and the participating nursing homes.
