PMAP/PGAMC Medical Education Fund September, 2002 Distribution Summary In response to ongoing concerns about the ability of Minnesota teaching hospitals and ambulatory training sites to continue funding clinical training for health professionals in a more competitive healthcare financing environment, the Minnesota Department of Health has distributed over $100 million since 1998 to clinical training sites around the state through the Medical Education and Research Costs (MERC) Fund. MERC funds are distributed annually using a formula that calculates grants based on average clinical training costs at each eligible site. Beginning in the fall of 2000, medical education ‘add-on’ funds began to be removed from capitated Prepaid Medical Assistance Program (PMAP) and Prepaid General Assistance Medical Care (PGAMC) rates and transferred to MERC for distribution directly to training sites. These add-on payments had historically been included in the capitated PMAP and PGAMC rates paid to health plans, but began being carved out of rates in October 2000 due to concerns that the funds, intended to offset higher costs at teaching facilities due to medical education, were not being passed through by health plans. Unlike the general MERC Fund, this is not new money but rather a transfer of existing funds which are distributed via a new mechanism. The first PMAP Distribution took place in June 2001. At that time, $4,441,818 that had been carved out between October and December, 2000 was distributed. Since that time, each distribution from the PMAP fund has contained six months worth of carved-out dollars. The September 2002 PMAP Distribution contained $15,417,746 that had been carved out between January and June, 2002. Upon receiving their grants, sponsoring institutions are required to pass funds through to the clinical training sites at which their students complete their clinical training. Over 400 training sites ultimately received grants, ranging from a low of less than $100 to a high of almost $2.6 million. Grants are based on a formula that equally weights relative teaching volume and relative public program volume at each facility. The table on the following pages shows the training sites that received grants of $10,000 or more from the September 2002 PMAP distribution. These sites received approximately 95% of all available funds. These figures represent the sum of all grants received by each site, many of which provide clinical training for students or residents from multiple programs. The largest recipient of PMAP funds was Hennepin County Medical Center (HCMC), followed by Fairview University Medical Center, Saint Mary’s Hospital, the Mayo Clinic and Regions Hospital. These five training sites alone accounted for more than half of the total distribution.
