These violations constitute grounds for disciplinary action under Minnesota Statutes, section 146B.08, subdivision 3, clauses (3), (9), and (10). Therefore, MDH is revoking your provisional body art establishment license as authorized by Minnesota Statutes, section 146B.08, subdivision 4. This decision will be made final and effective on the date you receive this notice. Under Minnesota Statutes, Chapter 14, you have a 30-day period from the date this revocation is received by you to challenge this decision and request a contested-case hearing.
