Dear Mr. Kellner:
Based on the facts and law in this matter as described in the enclosed Inspection Report, the Minnesota
Department of Health has determined your Body Art Establishment violates the requirements of
Minnesota Statutes, section 146B.02, subdivision 7 related to operating an establishment in a private
residence; section 146B.06, subdivision 2 related to health and safety standards; and section 146B.09
related to county or municipal land use ordinances. These violations constitute grounds for disciplinary
action under Minnesota Statutes, section 146B.08, subdivision 3, clauses (3), (9), and (10). Therefore,
MDH is revoking your provisional body art establishment license as authorized by Minnesota Statutes,
section 146B.08, subdivision 4.
This decision will be made final and effective on the date you receive this notice. Under Minnesota
Statutes, Chapter 14, you have a 30-day period from the date this revocation is received by you to
challenge this decision and request a contested-case hearing. Requests for a hearing should be made in
writing and include specific grounds for challenging the Department's decision. If you wish to request a
