How to Create a CFH (Child Family Health) Account
and Request Access to Forms on WIC Tools
The following are step-by-step instructions for creating a CFH Account and requesting access to a
form on the WIC Tools – CFH Forms and Applications page.
Go to the WIC Tools page. A link WIC Tools – CFH Forms and Applications has been added on the
HuBERT Information System page on the MDH WIC website.
Forms that require a CFH Account are labeled “A CFH Account is required”. Once you have a CFH
Account and are granted access, the link to the form will become enabled.

CREATE A CFH ACCOUNT
NOTE: If you already have a CFH Account, skip to the REQUEST ACCESS TO FORM section (page 2).
STEP 1: Click the Create a CFH Account link under How to access our tools.
STEP 2: Complete the Creating a CFH Account form. Read the disclaimer and click the button:

STEP 3: A Creating a CFH Account: Verify your information screen will open displaying the
information you entered. Review the information for any errors. Click the Continue >> button if the
information is correct. (If there are any errors, simply click the << Back to Form button, correct the
form and repeat Steps 2 and 3.)
STEP 4: A Creating a CFH Account: Verify your email address screen will open next informing you
that you will receive an email within 24 hours. The email will be from MDH_CFHCommunications
and verifies that the email address you provided is correct. A link or web address is provided in the
email to complete your registration.
Your registration must be completed within 24 hours of receiving the email. Otherwise, the
account request will be locked. If this occurs, please contact Sarah Mallberg for assistance at
[email protected].
Once you click the link on the email, your registration for a CFH Account will be complete.

REQUEST ACCESS TO FORM
STEP 1: Once you have a CFH Account, the next step is to sign in to request access to a specific
form. There are a couple of ways to do this:

You can immediately click the sign in link on the Creating a CFH Account screen that
opened when you completed your registration.
•
You can also sign in using the WIC Tools – CFH Forms and Applications link on the HuBERT
Information System page. To do so, simply click the Sign in link under How to access our
tools (2, pg. 1) on the Welcome to WIC Tools screen.
STEP 2: Complete the Sign In screen (Email and Security Code). Click the Continue >> button.
STEP 3: Complete the Sign In - Enter Your Password screen (Email and Password). Click the Sign in
button.
STEP 4: Click the Request Access link next to the form you want access to at the bottom of the
Welcome to WIC Tools screen.
STEP 5: Select the Grantee from the drop-down list on the Request Access screen.

STEP 6: If you select a single agency Grantee, one agency will display. If you select a multi-agency
Grantee, a multi-select listing of agencies will display. Select the agency or agencies you need to
be able to submit information for and click the Continue >> button.
STEP 7: A Request Access confirmation screen displaying the selected Grantee and
agency/agencies will open indicating that you will receive an email granting access to this tool
within 2-3 business days. Click the Submit button if the information is correct (click the << Back
button if not correct and repeat Steps 5 and 6).
STEP 8: The Welcome to WIC Tools screen will re-open informing you that your request has been
submitted to the State. Under Tools, the form will indicate that your access is pending.
The State will receive an email informing us of your request. We will verify your name and
Grantee/Agency, and grant you access to the form.
STEP 9: You will receive a Request Access Approved or Updated email from
MDH_CFHCommunications within 2-3 business days. The link/web address on the email will open
the same Sign In screens you used in Steps 2 and 3.
