Entering Data into REDCap
for Reporting Local Public Health Act Performance Measures
This guide describes how to enter data into the MDH REDCap 2015 Local Public Health Act Performance Measures project.
ENTERING DATA
1.
Log into the project entitled 2015 Local Public Health Act
Performance Measures.
2.
Click the Infrastructure: Capacity Measures from
National Standards (2015) Form in the left pane.

 Review the REDCap training guides for
assistance on logging in, navigating the REDCap
system, and filling out the data entry forms.
 Use the PPMRS instructions and worksheets to
compile your community health board’s data
prior to entering it into your REDCap project.

In the Incomplete Records dropdown menu, select the Record ID of the community health board for which you are
adding information.

Confirm you are entering data for the correct CHB.

Fill in the information for the selected CHB. As you respond to questions, more questions may appear depending
on your responses and the area of responsibility in which you’re working.
6.
Do not change the form status next to the Complete prompt—leave it as Incomplete.
7.
To save data click Save Record, Save and Continue, or Save and go to Next Form:
a.
Save Record saves the data and returns you to the main data entry screen of the Infrastructure: Capacity
Measures from National Standards (2015) Form. This is best to use when you need to enter or update
Data for your CHB.
b.
Save and Continue saves the data and keeps you on the current page.
c.
Save and go to Next Form saves the data and leads you to the next form, which is Infrastructure:
Minnesota-Specific Measures (2015).

ERROR MESSAGES
The following are potential error messages you may see while completing and saving a Data Entry Form. Follow the
guidelines for error message resolution below.
SAVE YOUR CHANGES?
WHAT IT IS
This error message appears if you try to leave a form before saving.
WHAT TO DO



Save changes and leave saves the data and takes you to the main data entry screen for the form.
Leave without saving changes takes you to the main data entry screen for the form without saving your data.
Stay on page does not save the data and keeps you in the same form.
SOME FIELDS ARE REQUIRED!
WHAT IT IS
This error message appears if you try to leave a form before providing a response to a required field.
WHAT TO DO


Okay keeps you from leaving the form so that you can enter data into the required field.
Ignore and leave record takes you to the main data entry screen for the form without saving data in the required
field. You may return to the form later to enter the required data.
