and 17 years of age or younger as of December 31 of the
measurement period
Event
At least one face-to-face well child visit (Well Child Visit Value Set)
with an eligible provider in an eligible specialty during the
measurement period
Denominator
Patients within the eligible population who had a BMI percentile greater than or equal to
85, as calculated by the Centers for Disease Control and Prevention’s formula

Numerator
The number of patients in the denominator who received BOTH counseling for nutrition
and counseling for physical activity during the measurement period.
Counseling for nutrition documentation must include a note indicating the date and at
least one of the following:
 Discussion of current nutrition behaviors (e.g., eating habits, dieting behaviors)
 Checklist indicating nutrition was addressed
 Counseling or referral for nutrition education
 Patient received educational materials on nutrition during a face-to-face visit
 Anticipatory guidance for nutrition
 Weight or obesity counseling
Counseling for physical activity must include a note indicating the date and at least one
of the following:
Required Exclusions

Discussion of current physical activity behaviors (e.g., exercise routine, participation
in sports activities, exam for sports participation)

Checklist indicating physical activity was addressed

Counseling or referral for physical activity

Patient received educational materials on physical activity during a face-to-face visit

Anticipatory guidance specific to the child’s physical activity

Weight or obesity counseling
The following exclusions must be applied to the eligible population:
 Patient was pregnant during the measurement period (Pregnancy V/Z Codes Value
