MINNESOTA IMMUNIZATION INFORMATION CONNECTION MIIC Data Use Agreement FAQ This document responds to frequently asked questions about completing the MIIC Data Use Agreement forms. What browser is compatible with the Data Use Agreement forms? Internet Explorer is the only browser that will consistently allow for an electronic signature to be entered and saved. If you do not have Internet Explorer installed, you can find the latest version at Microsoft Internet Explorer (http://windows.microsoft.com/en-us/internet-explorer/download-ie). What software will I need to use the Data Use Agreement forms? Adobe Reader version 8 or higher is needed to fill out the forms. You can download the free software Adobe Acrobat Reader (https://get.adobe.com/reader/). How do I submit the forms? To submit the forms, you must save the completed PDF to your computer and send it as an email attachment to the MIIC Help Desk at [email protected]. How do I do an electronic signature? 1 After completing the rest of the Agreement, click on the “Organizational Authorized Representative’s Signature” box on page 2. 2 If you already have a digital signature saved, move ahead to step 7. If you do not have a digital signature saved, set up a digital signature by selecting “A new digital ID I want to create now.” 3 Select “New PKCS#12 digital ID file.” (11/15) PAGE 1 OF 2 MIIC DATA USE AGREEMENT FAQ 4 Enter your name, organizational unit, organization name, and email address. 5 Select where you would like to save your digital ID and enter a digital ID password. 6 Select the signature you would like to use and enter your digital ID password. Check the “Lock Document After Signing” box. 7 Click “Finish.” Your new signature should be applied.
