Population served: Children (0 through 18 years) who fall within any of these categories: Population served: Uninsured and underinsured2 adults (19 years and older). ▪ ▪ ▪ Uninsured American Indian/Alaskan Native Covered by a Minnesota Health Care Program (MHCP) including: ▪ Fee-for-service Medical Assistance (MA) ▪ MinnesotaCare (MnCare) ▪ Prepaid Medical Assistance Plan (PMAP) ▪ Underinsured1,2 Providers: Must be enrolled in MnVFC. Providers: 180 sites were selected through an MDH application process in September 2016 to participate in this program through September 2019. Vaccine ordering: Participating sites will be able to order adult vaccines through MDH. Vaccines may be ordered using paper or via the Minnesota Immunization Information Connection (MIIC).
