HOW TO REQUEST A DEATH CERTIFICATE FROM THE FINALIZE FILING PAGE
1.
From the Select a follow-on action drop-down menu, click on ‘Request a death certificate’.
2.
Select the Issuance Office from the drop-down menu unless you’ve selected a default issuance
office.
3.
Use the Special Instructions text box to enter particular directives for the Local Issuance office.
4.
Click the Send to the funeral home box to populate the funeral home mailing information fields.
5.
Enter the first and last names of the funeral director or designated staff receiving the death
certificate(s). These are required fields.
6.
Complete the # of copies of certified fact of death or # of copies of certified cause of death
fields. Click the VA certificate box when requesting VA certificate(s). The VA copies are included
in the total number of copies ordered.
7.
Skip to step 9 if you do not wish to have certificates mailed to another location.

To have copies sent to a different mailing address in addition to the funeral home, click the Add
mail to button. Complete the Mail to information #{2} fields.

Click Proceed.
Review the Request summary.
a. Click Save to continue and the following message will display: “The request for death
certificate has been successfully submitted to <name of issuance office>.”
b. Click Modify request to make changes.
