HOW TO CORRECT A DEATH RECORD
Only the decedent’s personal and demographic information may be corrected by the funeral director or his/her
staff assistants. Corrections may be made to the demographic, legal and medical items on a death record within
one year of the date of death unless a death certificate has been issued. If more than a year has passed since the
date of death, or a death certificate has been issued, submit a Funeral Home Application to Amend a Death
Record. Medical information on the death certificate must be corrected by the Office of Vital Records if a death
certificate has been issued.

Click on the Death tab.

Select Search Death Record from the Tasks list.
3.
Enter the search criteria (e.g., first and last name of the decedent).

Click Search at the bottom of the screen. The page will refresh and display the search results on the lower
part of the screen. The death record summary at the top of the screen defaults to the first record in the
search results.

If the record summary displayed is the record to be corrected, proceed to Step 7.

If not, view the search results list; click the appropriate decedent’s name to select that record.

At the Select a follow-on action field, choose ‘Correct death record’ from the drop-down menu.

The Correct a death record page will open unless the record is not eligible for correction.
9.
Under Select reason for a correction, review the list of options in the Reason field drop-down menu.
10. Choose the reason; click Continue.
11. The page refreshes and displays the Decedent I node. Click the node(s) (listed below) that contain the data
field(s) to be corrected.
Decedent I - personal (identification) information
Disposition
Decedent II – residence, education, occupation and race
Place of death
Decedent III – marital, parents, and information
Medical Certifier
12. Make the correction(s); click Continue to save.
13. Click the Finalize Filing node.
14. The page displays the fields that were changed side-by-side with the original entries.
15. Verify the corrections.
16. Click Back or Cancel corrections to return to the record to make changes.
17. If the entries are correct, click File corrections at the bottom of the page. The page will refresh and display
“Death record successfully updated.”
