HOW TO ISSUE A DISINTERMENT PERMIT
Death Certificate Filed in Minnesota
1.
Click Search death record in the Tasks list on the Death tab.
2.
Type in the decedent’s first and last name (add other search criteria to narrow the results).
3.
Click Search.
4.
The Death record summary screen will appear.
5.
Click on the decedent’s name in the results list in the lower portion of the screen.
6.
In the Select a Follow-on Action drop-down menu, choose ‘Issue disinterment permit’.
7.
On the Issue disinterment permit screen (auto-populated with the decedent’s information),
complete the Reinterment information box. The type of reinterment selected will open
additional fields.
8.
Click Add legal representative to enter the name of the person authorizing the disinterment/reinterment of the deceased. A total of three legal representatives may be added to the permit.
9.
Complete Expected date of disinterment and Expected date or re-interment fields.
10.
Click Save. Permit added successfully will appear at the top of the screen.
11.
Click Print. Open and print the resulting PDF. In addition to the information added to the autopopulated screen, the permit contains the date that the permit was created and the permit
number.
Another option:
1. Click on the Death tab.
2. Under Tasks, select Disinterment Permits.
3. On the resulting Search Disinterment Permit page:
a. Create a new disinterment permit by clicking Create new disinterment permit.
i. Complete the required fields on the form displayed.
ii. Click Save.
iii. Click Print.
b. If the disinterment permit number is known, enter that number in the search field and
click Search.
i. Click Print.

Death Certificate Filed Outside of Minnesota
1. Click Disinterment permits in the Tasks list on the Death tab.
2. An empty Issue disinterment screen will appear.
3. Enter the decedent’s information, including the current interment location of the deceased.
4. Complete the Reinterment information block. Additional fields will display depending on type of
reinterment selected.
5. Click Add legal representative to enter the name of the person authorizing the disinterment/reinterment of the deceased. A total of three legal representatives may be added to the permit.
6. Complete Expected date of disinterment and Expected date of re-interment fields.
7. Click Save. Permit added successfully will appear at the top of the screen.
8. Click Print. Open and print the resulting PDF. In addition to the information added to the autopopulated screen, the permit contains the date that the permit was created and the permit
number.
