HOW TO REQUEST CREMATION AUTHORIZATION – UNFINISHED RECORD
1.
Select Enter death record in the Tasks list on the Death tab.
2.
Complete the basic information for death record fields. Click Save & Continue. The Decedent I
screen will appear.
3.
Click on Disposition. In the Disposition information pane, click the Cremation box.
4.
Click the down arrow to the left of the ME/Coroner field to view the list of medical
examiners/coroners for the county where the death occurred.
5.
Click on the ME/Coroner’s name. The page will refresh and auto-populate the ME/Coroner
information.
6.
Select the crematory from the Facility drop-down. The page will refresh and auto-populate the
required fields. If the specific crematory is not listed, select ‘Other’ and enter the crematory
information.
7.
In the Funeral home (Or Other institution) information pane, the Institution type, Establishment
name, and Funeral director fields may be auto-populated with information from the users’
profile if default a funeral home has been chosen. If different information should appear, use
the drop-downs to select the appropriate institution, establishment name, or funeral director.
8.
Click Save, then click Continue.

There is no chance to return to the pop-up question “Could the manner of this death be
Accident, Homicide, Suicide, or is this Death pending investigation or will the Cause of Death be
provided by Coroner/ME?” Click ‘Yes’ or ‘No’ to continue.
10.
Enter the first three or four letters of the physician’s last name into the Last name field. Click
Search. Select the physician from the drop-down to the right of the License number field. The
remaining fields on the screen will auto-populate.
11.
Click Save. The message Death record updated successfully displays.
12.
An email notification will be sent simultaneously to the ME/Coroner and the physician named as
medical certifier to notify both of the request for cremation authorization approval.
