Background
The Minnesota Department of Health is required to issue an annual report on community benefit
provided by Minnesota hospitals.1 This report describes community benefit provided in 2007 and
includes the following information from each hospital:









Cost of charity care (also referred to as “community care”);
Underpayment for services provided under state health care programs (the difference between
payments for services provided to public program clients and the cost of providing these
services);
Cost of conducting research;
Cost of providing medical education;
Community health services cost (examples include community health education, communitybased clinical services such as free clinics and screenings, health support services, and self-help
programs);
Financial and in-kind contributions;
Costs of community building activities (examples include economic development activities,
community support system enhancements, environmental improvements, and leadership
development training for community members);
Costs of community benefit operations (the cost associated with dedicated staff, and other costs
associated with community benefit strategy and operations); and
Costs of operating subsidized services (services that operated at a financial loss, such as trauma
services, burn units, and neonatal intensive care units).
For purposes of this report, community benefit does not include the cost of bad debt or underpayment
for Medicare services. In addition, community benefits reported are net of any offsetting revenues that
hospitals received for these activities.
Findings
In 2007, Minnesota hospitals reported approximately $636.7 million in community benefit, which
accounted for about 6.0 percent of total hospital operating expenses (see Table 1). As the single largest
category, state public program underpayments accounted for about 44 percent of total community
benefit, or $278 million (see Figure 1). Charity care and the cost of operating subsidized services each
represented about 16 percent of total community benefit, or about $100 million, respectively. The cost
of providing medical education amounted to about 14 percent of hospital community benefit, or $86.5
million.
The cost of operating community benefit programs, community building activities and health services
accounted for almost 8 percent of total community benefit in 2007, or $48.5 million.

Community Benefit Provided by Minnesota’s Hospitals in 2007
________________________________________________________________________________3
Community benefit varies by hospital characteristics, as shown in Table 2. For example, community
benefit at Minnesota hospitals as a percentage of operating expenses ranged from 3.4 percent in the
South Central Minnesota to 7.2 percent in the Northeast. On average, community benefit at rural
hospitals in 2007 accounted for 5.3 percent of operating expenses, compared with 5.8 percent at urban
hospitals.
In 2007, community benefit provided by government-owned hospitals accounted, on average, for 8.9
percent of operating expenses, compared with 5.3 percent at Minnesota’s private nonprofit hospitals.
The two small hospitals in Minnesota that operate as for-profit facilities provided community benefit
equal to one-half of one percent of operating expenses in 2007.
Table 3 shows community benefit by hospital. It is important to note that direct comparisons across
hospitals should be done with some caution. For example, not all hospitals were able to fully report
offsetting revenues for community benefit, and some hospital systems had difficulty allocating their
community benefit across individual hospitals.
