The Task Force will be involved in reviewing and selecting state priorities. The Task Force will be provided quarterly updates on the progress of the needs assessment. Early Childhood Continue a work group to assist the MDH in the development of a state Adolescent Health Plan by providing input on recommendations and generating actions steps for each area. The purpose of the plan will be to provide guidance and direction on activities to support adolescent health activities in Minnesota. Convene a larger group to recap the information gathered through state-wide listening sessions and focus on establishing next steps and strategies to address identified needs and priorities. Infant Mortality Reduction January 2015 to June 2015 Review Early Childhood system, with particular focus on where health fits in. Provide direction, guidance, and assistance with priority identification on issues related to early childhood. This includes input on planning for National Help Me Grow, Family Home Visiting, the Prenatal to Three initiative, Race to the Top, developmental and mental health screenings and other issues and gaps as they affect children’s healthy development. Adolescent Health Timeline January 2015 to July 2015 First phase of the Infant Mortality Reduction Plan will be provided to the Task Force Updates on strategies and action steps being developed by work groups for the second phase of the plan will be provided quarterly. Continue to support planning group in 2015. Convene larger group in mid-2015. Provide first phase in early 2015 and provide updates on second phase of the plan quarterly Community Engagement Develop a plan to gather community input on MCH issues, in addition to the traditional method of gaining input through consumer representative members Ongoing Task Force Monitoring Task Force Membership The Executive Committee will receive and review applications for any open positions on the Task Force. This includes members with terms expiring in January 2016. The Executive Committee and Task Force will also select a chair-elect for 2016 (to begin term in 2016). Task Force Executive Convene quarterly meetings of the executive committee Committee one month prior to the full Task Force meeting. Betty Hubbard MCH Solicit nominations for Betty Hubbard awards. Review Leadership Awards and recommend award recipients to the Commissioner of Health. Present the awards. 2016 Title V Block Grant Review 2016 block grant application and 2014 block Application and 2014 Annual grant annual report. Review state-specific measures. Report Develop 2016 Work Plan Brainstorm ideas for 2016 Work Plan and discuss priority areas for 2015 Work Plan with executive committee. Full Task Force reviews and approves 2016 Task Force Work Plan. Family Delegate Continue to support the role of the family delegate, including participation in the AMCHP Family Delegate/Mentor program. Children and Youth with Monitor the ongoing implementation of Special Health Needs recommendations from the CYSHN Strategic Plan. Coordinate activities with Task Force staff will maintain relationship with the State State Community Health CHS Advisory Committee (SCHSAC) and Office of Services Advisory Committee Minority and Multicultural Health (OMMH) staff. Task (SCHSAC), Rural Health Force members representing SCHSAC and OMMH Advisory committee (RHAC) share information at Task Force meetings and share and Office of Minority and information about Task Force with represented groups. Multicultural Health (OMMH) Submit quarterly updates from Task Force to SCHSAC. Invite OMMH Director to Task Force meetings. Member and Family Stories Receive information from Task Force members on their personal or professional stories. Program Updates Receive updates from MDH or community programs and partners on MCH/CYSHN-related activities. Legislative update of interest Receive updates on legislative activities specific to to Task Force maternal and child health and children with special health care needs of interest to Task Force. Work Group/Committee Updates Prematurity Task Force Continue to receive regular updates on the progress of the legislatively-mandated Prematurity Task Force. Preconception Health Work Continue the MCH Advisory Task Force Preconception Group Health Work Group to provide direction, guidance and assistance with priority identification on issues related to preconception health and life course. A draft strategic plan has been developed and will be finalized in early 2015. Pregnancy Risk Assessment Continue to convene the PRAMS Advisory Committee. and Monitoring System Minnesota PRAMS gathers state-specific information on (PRAMS) Advisory the health of mothers and infants in Minnesota. This Committee committee provides input to the MDH on using PRAMS data to address public health issues and to develop effective programs and policies. Children and Youth with Continue to inform the statewide CYSHN program and Special Health Needs help to identify gaps, assess services available to Parent/Youth Work Group March 2015 to June 2015 Timeline Early 2015 and ongoing. School-Based Health Centers How the MCHATF can get involved in some of the work being done around paid parental leave. What is MDH doing around supporting parents to be healthy parents? (Not just related to home visiting) Are there opportunities? Do we see this as a role? Could be a crossagency discussion with MDE. Parent-to-Parent Support for CYSHN and building the peer-to-peer relationships. How do we have a better understanding of what’s happening at the LPH level for children and youth with special health needs? National Help Me Grow project Title V Yearly update SCHSAC: Provide an introduction to the committee WIC: Provide updates on eligibility changes and the work that has been done regarding patient-centered services Education reform in Minnesota and health’s role in the schools: MnSIC, Generation Next, etc. Mental Health: gaps in mental health services, available services, school-based grants, pediatrics and adults Integration of maternal and child health and oral health
