2015 Title V Block Grant Needs Assessment



Title V Needs Assessment leadership team will select top state priorities based
on data, reports and stakeholder inquiries.
The Task Force will be involved in reviewing and selecting state priorities.
The Task Force will be provided quarterly updates on the progress of the needs
assessment.
Early Childhood



Continue a work group to assist the MDH in the development of a state
Adolescent Health Plan by providing input on recommendations and generating
actions steps for each area. The purpose of the plan will be to provide guidance
and direction on activities to support adolescent health activities in Minnesota.
Convene a larger group to recap the information gathered through state-wide
listening sessions and focus on establishing next steps and strategies to
address identified needs and priorities.
Infant Mortality Reduction


January 2015 to June
2015
Review Early Childhood system, with particular focus on where health fits in.
Provide direction, guidance, and assistance with priority identification on issues
related to early childhood. This includes input on planning for National Help Me
Grow, Family Home Visiting, the Prenatal to Three initiative, Race to the Top,
developmental and mental health screenings and other issues and gaps as they
affect children’s healthy development.

 Develop a plan to gather community input on MCH issues, in addition to the

This includes members with terms expiring in January
2016. The Executive Committee and Task Force will
also select a chair-elect for 2016 (to begin term in

Solicit nominations for Betty Hubbard awards. Review

Develop 2016 Work Plan
Brainstorm ideas for 2016 Work Plan and discuss
priority areas for 2015 Work Plan with executive
committee. Full Task Force reviews and approves 2016

Continue to support the role of the family delegate,
including participation in the AMCHP Family

School-Based Health Centers
How the MCHATF can get involved in some of the work being done around paid parental
leave.
What is MDH doing around supporting parents to be healthy parents? (Not just related to
home visiting) Are there opportunities? Do we see this as a role? Could be a crossagency discussion with MDE.
Parent-to-Parent Support for CYSHN and building the peer-to-peer relationships.
How do we have a better understanding of what’s happening at the LPH level for
children and youth with special health needs?
