ANNUAL CASE MIX ELECTION FOR
RESIDENTS WHO STAY 14 DAYS OR LESS
Dear Administrator:
This letter contains information regarding the Short Stay Election for short stay residents and the
management of the email addresses receiving electronic notification that Case Mix Classification
Notices, Checklists and Validation Reports are posted on the CMR Portal. Nursing facility staff
make elections and manage email addresses through the Minnesota Department of Health
(MDH) website. Your facility HFID and password along with instructions for accessing the
MDH website are enclosed.

 Please keep the enclosed instructions to manage email addresses for electronic
notification that notices, checklists and reports are posted on the CMR Portal.
 The HFID and password for the website are on the enclosed instructions.

Short Stay Election
Each year Medicaid certified nursing facilities must decide whether they want to complete
and submit an OBRA Admission assessment on all residents admitted to the facility
regardless of length of stay or payment source OR accept a short stay rate with a Case Mix
Index of 1.00 for all residents who stay 14 days or less. As a provider, you make this election
through a secure website and may choose one of these two options.
If your facility does not make an election by Monday, June 12, 2017, the facility must complete
and submit an Admission assessment (A0310A=01) for all residents admitted from July 1, 2017,
through June 30, 2018 [MS § 144.0724 Subd. 5(a)].

Nursing Facility staff will be able to access the MDH secure website beginning Monday, May
22, 2017. The deadline for the short stay election is 4:30 p.m., Monday, June 12, 2017.
Providers may change their election multiple times prior to the deadline. After June 12, 2017,
the short stay election choice is final and will remain in effect from July 1, 2017, through
June 30, 2018.

Description of Election Options –
Scenarios describing both election options are found in Appendix B (beginning on page 35) and
in Appendix C (on page 37) of the “Case Mix Classification Manual for Nursing Facilities,”

(A) – Facility Elects to Accept Short Stay (DDF) Rate for ALL residents who stay
14 days or less
When a facility elects to accept the short stay rate for all residents who stay 14 days or less,
including residents discharged return anticipated, Case Mix assigns the short stay (DDF) rate.
Facilities that elect the short stay rate for all residents who stay 14 days or less, must submit an
Entry tracking record (A0310F = 01) followed by either a Discharge assessment (A0310F = 10
or 11) or a Death in Facility tracking record (A0310F = 12). Facilities electing the short stay rate
do not need to complete and submit OBRA Admission assessments for residents who stay 14
days or less.
(B) – Facility Elects to Complete an OBRA Admission Assessment (A0310A = 01) for ALL
residents
When a facility elects to complete an OBRA Admission assessment (A0310A=01) for all
residents admitted to the facility, an Entry tracking record (A0310F = 01) and an OBRA
Admission assessment (A0310A = 01) must be completed for all residents regardless of their
length of stay or payment source.
PLEASE NOTE: All staff who schedule MDS’ and those who bill for a resident’s stay, need
to be aware of the facility’s election as it impacts the MDS’ they are required to submit and
determines what rate to bill.
Managing Email Addresses for Electronic Notification
Facilities may use the enclosed HFID and password to log in at any time during the year, to add,
modify or delete the email address of person/s receiving email notification that Case Mix
Classification Notices, Checklists and Validation Reports have been posted to the CMR Portal.
Please share the enclosed instructions with the CMR Portal Director or staff designated to update
the email addresses throughout the year.

**PLEASE KEEP THESE INSTRUCTIONS. Facilities may use the HFID and password to
log in at any time during the year, to add, modify or delete the email address of person/s
receiving email notification that Case Mix Classification Notices, Checklists and Validation
Reports have been posted to the CMR Portal. The CMR Portal users should receive email
notifications that checklists and notices have been posted.
Instructions:
1. To access the MDH secure website go to: https://pqc.health.state.mn.us/cmr-facility-options/
2. Enter facility HFID and PASSWORD on the login page.
3. Click “Logon”
4. On the election option screen, click on the “Radio” button to the left of your election option.
5. REQUIRED: Enter the name and title of person completing the election process. If not
completed, the facility election will not be accepted.
6. Enter an email address to receive confirmation that your election option was accepted.
7. Click “Submit”
8. The email address entered in step 6 will receive confirmation that the election was accepted.
9. On the next screen verify, edit or enter the email addresses of the person/s who will receive an
email when the Case Mix Classification Notices, Checklists and Validation Reports are posted
on the CMR Portal. The CMR Portal users should receive email notifications that checklists and
notices have been posted. A facility may provide up to five (5) email addresses. Facilities may
add, edit or delete the email addresses any time during the year by logging onto the election
website: https://pqc.health.state.mn.us/cmr-facility-options/
10. Click “Submit”
11. Receive screen confirmation of the current email addresses.

144.0724 RESIDENT REIMBURSEMENT CLASSIFICATION.
Subd. 5. Short stays.
(a) A facility must submit to the commissioner of health an admission assessment for all
residents who stay in the facility 14 days or less.
(b) Notwithstanding the admission assessment requirements of paragraph (a), a facility
may elect to accept a short stay rate with a case mix index of 1.0 for all facility residents who
stay 14 days or less in lieu of submitting an admission assessment. Facilities shall make this
election annually.
(c) Nursing facilities must elect one of the options described in paragraphs (a) and (b) by
reporting to the commissioner of health, as prescribed by the commissioner. The election is
effective on July 1 each year.
