MATHEMATICS HONOR SOCIETY
2015-2016 Application
The Paint Branch Mathematics Honor Society recognizes Paint Branch seniors and junior
for consistent excellence in mathematics.
All students that apply by the stated deadline and meet the following criteria are guaranteed
admission:
(1) Be a PBHS senior graduating with the class of 2016 or a junior taking AP Calculus BC
(2) Be currently enrolled in a math course through PBHS
(3) Have at least a 3.5 unweighted GPA for all high school credit math classes
(4) Have successfully completed Algebra 1, Geometry and Algebra 2
(5) Have taken and passed the Algebra HSA
If any of the above criteria are not met, students may still be admitted; however, admission is not
guaranteed.
INSTRUCTIONS FOR COMPLETING THE APPLICATION
 PRINT NEATLY using blue or black ink.
 Begin with Algebra 1 (even if completed in middle school) and continue to your most recently
completed course.
 List each semester separately.
 Complete all parts of the application, including the Math GPA calculation.
 Be sure to sign your application and get a signature from your parent/guardian as well.
 Return your completed application to Mrs. Kim Feher, Math Resource Teacher, in 2328 by
Friday, September 18, 2015.
Applicants will be notified of acceptance on or before Thursday, October 1, 2015.
A lunch reception will be held for honored students during LAP on Thursday, October 8, 2015.
