TI-83+ Calculator Lending Agreement
All algebra students are required to have a Texas Instrument (TI-83+ or TI-84) Graphing
Calculator by Sept 16th. We strongly encourage you to purchase a TI-83+ instead of
borrowing one. Your student will be required to have one in future math classes
throughout high school and in college.
Our inventory of calculators is less than the total number of students taking Algebra.
We will lend the calculators on a first-come first-served basis. If you choose to borrow a
calculator, complete this form and return it with the $45 deposti to your student’s math
teacher by September 11th.
Please read and initial the following conditions with your student:
______ We will replace the 4 AAA batteries that are required to operate the calculator
at our own expense as needed.
______We will keep the calculator in a secure location at all times.
______ We will return the calculator in good working condition.
______ We are responsible for any damage or loss to the calculator. If lost, stolen or
damaged, our deposit will not be returned and we will pay an additional $45
replacement fee.
To pay your deposit, please make check for $45 payable to White Oak Middle School.
