payable to Lifetouch. No post-dated checks. Please be sure to include name,
address, phone #, date and signature on check. When you pay by check,
you authorize us to process the payment as a check transaction, or to use
information from your check to make a one-time electronic fund transfer from
your checking account. A service fee may be charged on returned checks.
Prices include tax where applicable, except for residents of MD and PA, who
are required to add 6% tax to their order. Returned check fee is $25.
