MAJOR FUNCTION
Responsible for developing, coordinating and implementing tasks that relate to the success of the
Criminal Justice, Mental Health, and Substance Abuse (CJMHSA) Reinvestment Forensic Intensive
Case Management (FICM) Project. Plans strategies to ensure deliverables are completed successfully
and on time. This position manages reporting aspects of the operational, clinical, and financial
portions of the project and liaisons with project team members. This position is the face of the FICM
project in the community and will be the central point of contact for the project including receiving
referrals. This position requires the use of independent judgment in setting priorities, coordinating
work, handling multiple responsibilities, and the ability to manage time while working with
deadlines, and interacting with team members on a daily basis. Create and execute project work plans
and revises, as appropriate, to meet changing needs and requirements. Works under the direction of
the Community Health Care Planner and provides organizational support to key team members as
needed.
ILLUSTRATIVE DUTIES
Establish and maintain communication and constructive and cooperative working relationships with
and among partners in the project. Collaborates with key stakeholders to implement and provide
ongoing oversight and quality improvement activities of the project. On a monthly and quarterly
basis, collects and analyzes data, and evaluates and reports project performance measures and
outcomes, including a plan for dissemination of information. Plans and coordinates CJMHSA
Planning Council meetings. Prepares and submits project reports to Division leadership, the
CJMHSA Grant Planning Council, Florida Department of Children and Families, the CJMHSA
Technical Assistance Center and other groups as required. Keep supervisor informed of pending
activities and the status of assignments.
Collects, analyzes, organizes, and presents data in an actionable format which supports decision
making to meet goals and objectives of the CJMHSA grant. Compiles and summarizes various
reports pertaining to the CJMHSA grant. Coordinates meetings, prepares agendas, meeting minutes
and presentations. Participates in collaborative planning activities, summarizes outcomes, and
identifies next steps. Highlights and eliminates process duplication.
Responsible for promoting the project in the community and must be proactive in developing and
streamlining the referral process to meet enrollment goals. Receives and processes referrals and
assigns cases to case managers. Manages incidental funding requests from case managers to insure
appropriate use of funds. May have occasional contact with clients. Responsible for collecting and
incorporating contributions from key team members and integrates feedback to project plans. Ability
to track changes and produce updated reporting documents. Responsible for compiling and utilizing
all resources required for completion of the project. Tracks and reports outcomes. Maintains an
overview of all reporting components of CJMHSA grant and effectively communicates progress in a
timely manner. Develops and maintains detailed reports and schedule of reports. Ensures all
CJMHSA grant reporting and monitoring documents (hard and virtual) are complete, current, and
stored appropriately. Investigates, researches, composes, prepares and completes information

memoranda, various reports and correspondence. Identifies, develops, and maintains procedural
guidelines as identified by Florida Department of Children and Families. Responsible for data entry
and report generation using health plan specific software application. Works with implementation
team and CJMHSA Grant Planning Council to ensure grant reporting requirements are submitted
accurately and on time. Assist Community Health Care Planner with communication strategies as
needed. Performs related duties as required.
KNOWLEDGE, ABILITIES AND SKILLS
Must be proficient in Microsoft software applications including Word, PowerPoint, and Excel. Must
be willing and able to learn CareScope electronic records system upon hire. Ability to perform
independently or as a member of multiple team efforts across diverse groups. Ability to manage
multiple tasks at one time. Ability to identify, research, investigate and verify challenges as well as
recommend solutions or alternatives. Must have the ability to troubleshoot and problem solve using
critical thinking. Ability to perform creative writing tasks. Must be self-motivated. Must have the
ability to communicate effectively in a professional manner, both orally and in writing and have
excellent organizational skills. Ability to network and build relationships within the community to
achieve project success.
MINIMUM QUALIFICATIONS
Graduate of an accredited college or university with degree in health care administration (Master’s
preferred) or a related human services field including criminal justice, mental health or substance
abuse. A minimum of 5 years of related experience in the fields previously mentioned which includes
project management/coordination and 2 years of supervisory experience.
Knowledge of HIPAA compliance regulations. Emphasis on problem determination and resolution,
and the ability to use various hardware, software, network, and communication systems. Must have
strong knowledge of computer systems and report writing.
Must possess a valid Florida driver's license at the time of employment.
A comparable amount of related training and experience may be substituted for the minimum
qualifications, with the exception of having a college degree.
SPECIAL REQUIREMENTS
Must provide and utilize personal transportation in daily travel to remote sites to perform assigned
duties and obtain personal injury protection insurance. Must have a phone within thirty (30) days of
employment in this classification as employee. Must be available to work at various locations
throughout Polk County.
Must be available to work special assignments such as shelter duty during emergency activation of
special needs shelters within Polk County.

SPECIAL PREFERENCE
Prefer applicants with knowledge and ability to use Health Information Technology, Electronic
Medical Records, and other applicable programs. Have a background and understanding of how to
work with individuals involved in criminal justice, mental health and/or substance abuse programs.
