Q&A: What Makes a Community Clinic Different?
The Arthur Nagel Community Clinic is one of only 1,200 community clinics in the U.S. The one thing they all have in common is that they are primarily funded by the community they serve, rather than patient fees. To help people understand what
their Community Clinic is and does, here are answers to some of the questions the staff and Board members have been asked:
Q: Where did the Arthur Nagel Community Clinic come from?
A: Most community clinics were started by another nonprofit corporation, an individual, or a group of individuals. With
the Clinic, it was a combination of all three. The Clinic was built by Helping Hand, the local crisis center in Bandera County, on
their “Village of Hope” property in 2007. Arthur Nagel of Kerrville, a former city councilmember, made the first gift for the Clinic
in 2006, $150,000 to build the shell of the Clinic building. Four committees of volunteers then raised the rest of the money needed for the Clinic and planned its construction, furnishings, and equipment. An independent Clinic Board of Directors was elected
to operate the Clinic as a separate entity from Helping Hand.
Q. What does the Clinic do?
A. The Clinic is a medical clinic whose services include diagnosis and treatment for non-emergency conditions, chronic
illnesses, injuries, infections, cancer screenings, sexually transmitted diseases, and referrals to other healthcare professionals. The
Clinic provides both Well Woman and Men’s Wellness exams. A volunteer licensed therapist is available to help patients deal with
mental health issues, plus we do send patients to the dentist for pain and infection. The Clinic has developed a network of 36
specialists to whom it can refer patients who need more care than the Clinic can provide.
Q. Who decides what the Clinic does?
A. The Clinic Board of Directors is responsible for all decisions relative to the operation of the Clinic. Bruce Hanks is the
Clinic’s Executive Director, and he makes recommendations about administrative initiatives or changes to existing procedures to
the Clinic’s Board of Directors through Lon Yaeger, the Board President. K. Irene Stone, former Executive Director, is the new Director of Development and oversees fundraising for the Clinic. We have three Nurse Practitioners – Debbie Zacheus, RN, FNP-C,
Sharon Wright, FNP-C, and Tom Mackey, PhD, FAANP – who work under the direction of the Clinic’s medical director, Saleh Jafaar,
MD, plus we are fortunate to have a part-time Gynecologist, Sandra Haldeman, MD, to oversee our Well Woman Program (see
page 5).
Q. Can anybody be a patient at the Clinic?
A. No. The Clinic’s services are reserved for low-income families who don’t have health insurance. These are people who
have no other access to healthcare in Bandera County. There are approximately 4,200 residents in Bandera County who fall into
this category. Resources of the Clinic are stretched thin just trying to keep up with the medical needs of this group of Bandera
County residents.
Q. Why is it called a “free clinic?”
A. The Clinic is a free clinic because it does not charge a fee for its services. Donations are requested from patients, however. Patient donations are important to the Clinic, but even more important to the patient. Most patients want to contribute
something toward the cost of their care. But all patients receive treatment at the Clinic whether or not they are able to make a
donation.
Q. How can I find out if I qualify to be a Community Clinic patient?
A. The Clinic accepts patients who have no health insurance and whose household income is less than 200 percent of the
Federal Poverty Guidelines. An example of what that means is that a family of four would qualify with an income of less than
$47,700 a year. The best way to find out if you qualify is to call the Clinic at 830-796-3448 or check our website at
www.NagelClinic.org. You can also register online at our website.
Q. How can I make a donation to the Clinic?
A. Donations are vital for the Clinic to be able to continue to care for the underserved population of Bandera County.
Patient donations account for less than five percent of the Clinic’s operating costs. In 2013, the Clinic received $18,775 in patient
donations and but had expenses over $400,000. As you can see, it takes a lot of effort and dedication from individuals, businesses, churches, and foundations for the Clinic to stay open and provide free medical care. That is why we value every donation and

has a list of items we can use for our patients’ care and education.
Q. Does my donation only help the patients of the Clinic?
A. The Clinic is committed to reaching out to the Bandera County community. With our completed $450,000 building
expansion, we have more opportunity to reach more people within the community and help them lead healthier lives. We are
starting “Eating Healthy ” cooking classes in our teaching kitchen, and “Living Healthy” exercise and mental health workshops in
the Education Room. These classes will be open to the community. We believe that by promoting healthy lifestyle choices to
people within our community, we can indirectly help our patients. After all, if the family member of a diabetic does not know how
to cook nutritious, beneficial meals, how can we expect our diabetic patient to eat healthier and maintain a normal glucose level?
As you can see, a gift to the Clinic can have a huge effect on the overall health of both our community and the entire
county. Together, we can make Bandera County healthier!

Clinic Hours Open: Tuesdays, Wednesdays, Thursdays – 8:00 a.m. to 4:00 p.m.
Patient Criteria:
1. Prospective patients who are eligible for Medicare, Medicaid or CHIPS; government insurance, including VA; or have private health insurance, will not qualify for Clinic services. Failure by a prospective or current patient to inform the Clinic
that they have insurance can result in the patient being liable for all visits, medication, and lab expenses, including fees
associated with referral visits and tests. Patient must also reside in Bandera County.
2. Patients must be re-certified annually to be eligible for Clinic services. Household income may not exceed 200% of 2014
Federal Poverty Guidelines. (See chart below.)

All patients are asked to make a contribution to the Clinic to help pay for their healthcare.
We do not refuse treatment if the patient is not able to make a contribution. But the patient needs to know that our major funding
sources pay attention to what our patients contribute. They typically ask for the number of patients who contribute. So any amount
donated—whether five dollars or twenty dollars or more — improves the Clinic’s chances of receiving continued support from major
funding sources.
Appointment Procedure:
Prospective patients fill out an application to see if they qualify for Clinic healthcare services (this can be done online at
www.NagelClinic.org . Once approved, they can call 830-796-3448 to make an appointment. (Please note: an appointment is not
