I am inviting you to participate in a research study. Involvement in the study is voluntary,
so you may choose to participate or not. I am going to explain the study to you.
Please feel free to ask any questions that you may have about the research; I will be
happy to explain anything in greater detail.
I am interested in learning more about your family’s resources and needs. You will be
asked to answer survey questions that include education, employment, housing,
healthcare, and other basic needs. This will take approximately 30 - 45 minutes of your
time.
All information will be kept anonymous. This means that your name will not appear
anywhere and no one except me will know about your specific answers. I will not ever
share your name with anyone else about how you answered this survey.
The benefit of this research is that you will be helping the East Texas Human Needs
Network to better understand our community needs, the resources available, and the
services that are still needed. This information will help us to work together to improve
services that are needed the most.
There are no risks to you for participating in this study, and no penalty for not
participating. Our services to you will not be affected in any way if you choose to
participate or not. If you do not wish to continue, you have the right to stop, at any time.
Before we begin, I want to make sure that you understand the information I have told you
