COS Research Advisory - Data Needs Questionnaire
Dear colleague,
You are invited to participate in this questionnaire, which provides you an opportunity to share
your thoughts on improving the use of data and analysis throughout the District. Please take a
few minutes today to respond. Your participation in this survey is voluntary and no tangible
compensation will be given for participation.
Please note that your responses will be kept confidential. Survey results will only be reported in
the aggregate so that the privacy of the respondent is protected. The results will be analyzed by
the Office of Research, Planning and Institutional Effectiveness, and will be forwarded to the COS
Research Advisory Group. The results may also be used by the appropriate COS offices and
leadership to improve COS. This questionnaire will close on May 23, 2014 at 5:00 p.m. If you
