St. Luke’s Hospital earns best hospital in the corridor for the fourth time
It’s that exciting time of the year again when the best businesses in Iowa’s Creative Corridor are honored,
as chosen by the Corridor Business Journal's (CBJ) readers and UnityPoint Health - St. Luke's Hospital has
been voted best hospital in the corridor for the fourth time!
In June, the CBJ asked subscribers and the general public to vote for their favorites in 50 categories ranging
from the best professional firms to the best place to get a drink after work. Entries were taken by write-in
vote, meaning any company can win.

St. Luke’s strives to provide a healthy work environment, Mr. Townsend said, adding that the Cedar Rapids region of
UnityPoint Health’s eight major regions across Wisconsin, Illinois and Iowa has the highest associate satisfaction.
“I think when you have people who feel they are engaged and respected, they will provide better patient care, and we are
very proud of the fact that we have the best associate satisfaction within the entire system,” he said.
All 3,700 associates in the Cedar Rapids region continually challenge themselves to improve their performance year after
year, he said. “We are always trying to get better tomorrow than we are today.”

T.H.E. Care Model: 2016 priorities, recent
accomplishments and 3rd quarter activities
T.H.E. Care Model is the UnityPoint Health strategy to deliver on our brand promise of Care
Coordination. It was designed to solve problems our patients and providers regularly encounter in
our current health care processes. A foundational element of this work is the ability to risk-stratify
our patients to provide the right service at the right time to the right patient. Today we are happy to
share some key accomplishments towards our overall goal.
July Happenings
UnityPoint Health named Most Wired for the fifth consistent year
American Hospital Association’s (AHA) Health Forum has named UnityPoint Health a “Most Wired” Health System for
the fifth year in a row. Grundy County Memorial Hospital, a UnityPoint Health Community Network Hospital, has also
received the honor. According to the results of the 18th Annual Health Care’s Most Wired survey, technology is improving
the efficiency of care delivery and creating a new dynamic in patient interactions. Most Wired hospitals are using
telehealth to fill gaps in care; provide services 24 hours a day, 7 days a week; and expand access to medical specialists.
This year’s results show:
• The top three telehealth services offered in hospitals are consultations and office visits, stroke care, and
psychiatric examinations and psychotherapy.
• Stroke care is the most rapid growth area for telehealth services up 38 percent from 2015, as evidence-based
studies emphasize the time urgency of stroke care.
• More than 25 percent of hospitals use internet-enabled monitoring devices for chronic disease management of
congestive heart failure, diabetes and heart disease.
Our patient portal, ‘My Chart,’ is a valuable tool that allows us to connect quickly and easily with many of our patients
online - their preferred manner. UnityPoint Health is focused on these efforts, and others, in order to excel at the triple
aim of providing patients with quality care and optimal experience at a lower cost.

Paging Physicians
Sleep Rooms
St. Luke’s has received information from physicians about being paged
back to only a 4-digit number. When outside the hospital it is impossible
to know what prefix to use. We have shared this information with
associates at St. Luke’s but also wanted to share with providers when
paging a colleague. It is very important that those paging associates and
physicians enter all 7-digits. Below is a reference chart with prefixes in
case you only receive 4-digits in the page.
866 – for extensions 1000-1999
369 – for extensions 5100-5199 and 7000-8999
368 – for extensions 5500-5999
286 – for extensions 4300-4399
558 – for extensions 4800-4999
861 – for extensions 6700-6999
Any provider in need of a sleep room at
St. Luke’s while on-call should contact the
House Supervisor via the St. Luke’s operator.
The House Supervisor will be able to identify an
area for the provider to sleep for the evening.
Any extension not in the above ranges it is an internal number only and
cannot be dialed from outside.

Pokemon Concern Addressed
To ensure our patient confidentiality is not
threatened due to the new Pokemon game,
St. Luke’s has physically/virtually surveyed
the hospital to ensure we did not house any
Pokemon or Gems, contacted the developers by
filling out a form to exclude our campus, and will
continue to monitor on an ongoing basis.
• The Risk Stratification process has been enhanced to enable a more robust methodology
to capture claims, EHR, and chronic diseases to determine low, rising, and high risk. The
methodology has been validated by a small group of UnityPoint Clinic providers as they review
the results for their panel of patients. We are now ready for all UnityPoint Clinic patients to be
risk stratified and this information will display in Epic and on the Common Care Plan. We are excited
about the ability to use this risk stratification as it will ensure that we provide the right services to the
right individuals based on their needs.

• Patient Self-Scheduling Online will be piloted in the UnityPoint Clinic Family Medicine – Altoona site and in the Madison
region, via MyUnityPoint. Our patient portal. UnityPoint Clinic, the Call Center, the Patient Portal, and IT have worked
together to consolidate scheduling templates to enable both online and centralized scheduling.
• Centralized Scheduling will also be piloted in the Family Medicine - Altoona clinic following the patient self-scheduling
implementation (September). Centralized Scheduling redirects calls from the clinic for future appointments to the Call
Center, allowing UnityPoint clinic staff to focus on their patients.
• The Common Care Plan has been built to capture and display patients’ chronic conditions, personal goals, social
determinants, barriers to care, medication lists and common screening tools. Proof of concept testing was done in two clinics
to obtain provider feedback prior to piloting. In addition the build was vetted with Epic experts for content and display. Epic
leadership endorsed the content for population health management and offered suggestions for display. The Common Care
Plan will be auto-populated by other upstream documentation. The longitudinal care plan is now ready for pilot in the Cedar
Rapids region beginning July 17.
• Ambulatory care coordinators will be the key stakeholders. To ensure efficient care coordinator documentation and
auto-population of information into the Common Care Plan, a new flowsheet has been developed for use by care
coordinators. Education and training on this tool will begin June 28. Initial

• NOTE: The Common Care Plan will be available at the end of June. Cedar
Rapids will receive training and pilot usage for high risk and top tier rising
risk patients July 17 – September 30. Other regions may see the Common
Care Plan within a patient record but there is no expectation for completion
or utilization until wider deployment and training occurs in October.
The accomplishments thus far reflect the commitment and effort of many individuals

Epic Update
Rounding Navigator
The Rounding navigator now shows 24 hours’
worth of vitals for ICU patients and 72 hours’
worth of vitals for non-ICU patients.
Close the Red Door
To prevent HIPAA violations through
unauthorized access; please remember to
log out of Epic when leaving a computer
work station. This is best accomplished with
Badge Out activity or “closing the red door”
located on the upper right hand corner of
the screen.

There is now a smart text available for brain

death. It is called UPH Physician Brain Death
Diagnosis. To access this template, select the smart text search box
(next to the butler), type in brain death and select the UPH version for
a templated note.
Results Release to MyUnityPoint
Most test results are automatically released to patients active on the
MyUnityPoint portal. Meaningful Use allows for providers to prevent
the releasing of results on a result by result basis at the patient level.
Providers can now do so at their discretion using a new button in the
In Basket or the MyUnityPoint Results Release activity. This change is
coming as part of an Epic Special Update on July 17.
Bed Request Order
In the Bed Request order, a hard stop has been added to the Telemetry
required field.
Update to Medication Order Composer
When placing an order for a new medication, providers will see three
days’ worth of scheduled time. This was approved by the Inpatient
Physician Advisory Committee.
The denials management nurses would like to
thank all the physicians who so kindly agree
to help with the peer to peer evaluations for
Inpatient Level of Care Denials. It helps so
much when the doctor caring for the patient
talks with the insurance company reviewing
physicians. This process helps ensure the
appeals process is successful.

The Cancer Risk Assessment and Genetics Clinic at the
Helen G. Nassif Community Cancer Center provides
individual counseling, risk assessment, genetic testing,
and surveillance recommendations for your Individuals.
These services are provided by nurse practitioners with
expertise in oncology, counseling, and genetics. The
nurse practitioners have a combined genetics experience
of over 20 years.
Patients that may benefit from the Cancer Risk
Assessment and Genetics Clinic include:
• Individuals with a strong family history of any cancer.
• Individuals who have been diagnosed with cancer
under the age of 50.
