ORGANISATIONAL PROFILE
Community Care Services - Central Coast Ltd (CCS) is a leading community enterprise for the
provision of Dementia & Community Services on the Central Coast.
Originally an auspice of Gosford City Community & Information Service Limited with programs
commencing in 1986 CCS became an independent incorporated Association in 2001 changing
its structure to a Public Company Limited by Guarantee in 2007.
CCS has Public Benevolent Institution (PBI) status and Charitable Fundraising Authority
(CFN/21515) in NSW.
CCS conducts a range of programs funded at Federal & State levels to assist frail aged,
persons with dementia and their carers and people with a disability. Programs aim to assist
consumers to maintain a quality and healthy lifestyle thereby retaining their independence both
within their home and a wider community environment with the aim of preventing premature
admission to residential aged care.
Program coverage includes both Gosford & Wyong local government areas and includes:

Services are predominantly provided in a client’s home or community settings or in our three
Dementia Specific Social Clubs - Gosford, Woy Woy and Gorokan. Our Gosford centre is
commonly known as Geoff Wright Cottage (GWC); this centre has been in operation since
1986. Our Woy Woy facility, known as the Yaringaa Centre opened in 2009 whilst our Gorokan
facility, Beryl’s Place, opened in July 2012.
CCS head office is located at Unit 1 / 8 Yandina Rd, West Gosford.
The overall accountability of CCS rests with the voluntary Board of Directors, comprising a
Chairperson and up to six Directors. With a formal governance charter in place, the day-to-day
operations of CCS is the responsibility of the Chief Executive Officer (CEO). The CEO is
supported strategically & operationally by a Leadership Team comprising Programs Manager
and four Team Leaders - Co-ordination Unit; Dementia Services Unit; Memory Lane & CVS
Unit and Administration & Payroll Unit.
CCS has an annual turnover of approximately $3.6M and currently employs 70 staff (F/T, P/T
& casual) and is supported by 80 volunteers. Each week our programs support and assist in
excess of 950 clients and their carers.

The majority of funding for CCS comes from what was formally referred to as the Home &
Community Care (HACC) Program - a joint Commonwealth/State initiative. Resulting from
change in Government policy and implemented from July 2013 bulk of funds (92%) now come
from the Australian Government - Dept. of Social Services under the Commonwealth Home
Support program. The balance of funds come from NSW Family & Community Services –
Ageing, Disability & Home Care. Additional recurrent funds are also received from the
Australian Government – Dept. of Social Services under the National Respite for Carers
Program (NRCP) and Community Visitors Scheme (CVS).
CCS recently launched a “fee for service” community enterprise Blue Sky Home Solutions in
partnership with another community provider Vietnam Veterans Keith Payne VC Hostel.

CHAIRPERSON’S REPORT
It is with pleasure that I present the Chairperson’s Report, on behalf of
the Board of Directors for 2013-2014. The following key results were
achieved in alignment with the 2010-2014 Strategic Plan.





Community Care Services - Financial Sustainability and Growth
Community Care Services was successful in securing funding in
several areas this last financial year, including
Success in the 2013 HACC Funding Round – Annual Recurrent Grant Amount
$581,479
Securing an additional $63,000 Equal Remuneration Order (ERO) funding, applied
over three years, which should now flow through to 2018
Securing an additional $10,000 of funding through ADHC from the Sector Planning
Framework
Securing funding through the State Training Services for the amount of $24,000
Securing funding through Central Coast Medicare Local for $5,000
Overall, for the 2013-2014 financial year Community Care Services achieved an Operating
Surplus of $138,619 and resulting in net accumulating funds of $515,351 (2013 - $376,732).
Community Care Services - Quality Initiatives
There were three key strategic areas of quality development this year. Firstly Community
Care Services partnered with Macquarie University, with a mentoring project team from the
Faculty of Psychology, to create and carry out a survey into the needs of male carers whose
partner has dementia. Secondly, Community Care Services with the support of an $18,000
grant from the Dementia Collaborative Research Centre (DCRC), University of New South
Wales, undertook research on the Memory Lane Restaurant and submitted a paper for
publication. Finally, the implementation of a Quality Management System commenced with
the view of obtaining third party accreditation against the disability standards and ISO
Certificate in 2014-2015.
Community Care Services - Public Profile Initiatives
Linking to the community, including sourcing potential volunteers, keeping our politicians
informed of what we are achieving and creating and maintaining successful networks with
related organisations is vital for the strategic profile of our organisation. Community Care
Services has been successful in many ways in promoting our public profile such as holding
volunteer recruitment events and participation in DAS Unit - Dementia Awareness campaigns
across the Central Coast. Links were also established and strengthened with newly elected
members for Federal seats of Robinson and Dobell and current member for the Federal seat
of Shortland. Our links with Wyong Council were maintained leading to an offer of a formal
lease of the premises – Beryl’s Place (subject to ongoing negotiations). Key alliances were
maintained with Central Coast Community Transport Ltd and Meals on Wheels Central Coast
Ltd. Whist new alliances have been forged with Vietnam Veterans Keith Payne VC Hostel, Woy
Woy Community Aged Care Ltd and Samaritans.
Importantly, the organisation’s public profile was strengthened by the review of the Staff Dress
Code / Uniform Policy and the introduction of corporate uniforms for all staff. The smart
uniforms have enabled the organisation to be represented professionally out in the community.
As well as this, a video produced in conjunction with Sydney TAFE, ‘Dementia; a Promise

Kept’, was uploaded to YouTube, it is a video of the personal story of one client and his carer
and shows Community Care Services in a very positive light.
Community Care Services - Organisational Capacity
Many initiatives were undertaken in 2013/2014 to support and increase the organisational
capacity of Community Care Services, particularly in the area of IT development, remote
access technology and the refurbishment of one of our Social Clubs Geoff Wright Cottage in
2012-13 which has now allowed average participation rates to increase from 12 to 16
participants per day.
In the area of staff training and development, two key initiatives were undertaken both funded
by State Training Services. The first initiative was a staff training partnership established with
TAFE Gosford which enabled access to a variety of competency based training programs for
our staff. The second initiative was to enable professional development opportunities for the
organisation allowing a number of staff to complete Certificate lll in Community Aged Care.
New Initiative – 2014 / 2017 Strategic Plan
During the year the Board and Senior Leadership Team conducted a number of strategic
planning workshops determining a planned vision towards 2020 and a documenting a Strategic
Plan towards 2017.

Due to personal reasons, Mrs Debra Paterson was not able to complete her term as
Chairperson handing duties to our Deputy Chair, Mrs Annette Czerkesow in March 2014. Mrs.
Czerkesow has recently accepted a senior management position in Queensland and so has
also not been able to complete her term as Chairperson.
At this time, I would like to welcome Mr Chris Golden to the board and thank Peter and the
Board for the opportunity which was provided to me. I have benefited from their mentorship
and the knowledge gained and would like to wish the Board, staff and organisation all the
very best for the future.

CHIEF EXECUTIVE OFFICER’S REPORT
BUSINESS MANAGEMENT
Whilst meeting all our contractual obligations to our funding bodies (State
& Commonwealth), CCS achieved a surplus of $138,619 for the year and
increased total Members Funds to $515,351 being a 37% increase from
the previous year (2013 Members Funds of $376,732.)
With such a positive result, I am pleased to report that our Not-for-Profit Financial Health Check
barometer continues to move in an upward direction. This year’s reading of 33.2 / 50 is a 16%
improvement on last year’s result of 28.7 / 50.
Financial performance was significantly boosted during the year with success in the 2013
Commonwealth HACC Funding Round. CCS was awarded an additional $581,479 in recurrent
funds which translates to an additional 4,000 hours of Social Support; 6,020 hours of Domestic
Assistance; the introduction of a new service type Personal Care with 2,660 hours; and ability
to operate Beryl’s Place, 5 days per week.
Recurrent State & Federal grants awarded to our organisation now total $2,382,629 with 64%
of these grants designated dementia specific in their application.

PROGRAM DELIVERY
A full description of programs delivered are noted at pages 18 to 20
Success in the 2013 HACC Tender Round has resulted in approximately 30% increase in hours
of support that we can now provide to clients across the Central Coast and expanded our
dementia specific social club program with all three centres now opening Monday to Friday and
the possibility of alternate Saturdays if demand requires it.
Our expanded programs are now almost at full capacity and it is pleasing to report CCS is
supporting approximately 650 clients with excess of 39,000 hours in our In Home program and
approximately 150 clients with excess of 72,000 hours in our Dementia Social Club program.

Whilst numbers have dropped slightly from last year, we continue to support the Veteran’s
community across the Central Coast with 155 veterans or their war widows receiving
assistance with domestic assistance & personal care and/or carer respite.
A further 25 residents from various nursing homes also continue to receive a regular weekly or
fortnightly visit by one of our friendly and cheerful volunteers.
Our Memory Lane Program – Doing Things Together continues to attract new participants. A
new service was added this year – Men’s Tucker Workshop. This program combines cooking
classes for male carers with the dining experience of our Memory Lane Restaurants. Subject
to demand, this program is held monthly and allows a small group of male carers to learn the
fundamentals of menu & ingredient preparation and cooking & plating up a meal. Whilst the
men are in the kitchen with our chef their partners with dementia are pampered with a facial
and/or hand/foot massage. The couples then enjoy a restaurant atmosphere dining experience.
This programme was established with financial assistance from Central Coast Medicare Local
Network.
We are also very appreciative for the continuing financial assistance of Wyong Rugby Leagues
Club Group, Halekulani Bowling Club, Erina Rugby Leagues Club and the Rotary Club of
Northlakes Toukley for their continuing support of our Memory Lane programs and dementia
specific Social Clubs.
CCS has recently partnered with Samaritans –Older Parent Carer Program to launch a
Tucker Workshop for People with Disability

for People with Disabilities. This has
been an innovative collaboration
between historically different services.
The program whilst providing respite for
carers offers an opportunity for
participants to gain kitchen and cooking
skills, learn about nutrition and meal
planning, and develop friendships &
informal social networks. It has been an
extremely successful program.

The developing of relationships between
CCS and Samaritans Older Parent Carer
Program also led to a Carers – ‘Doing
Things Together’ Forum which was held on
9th October during Carers Week 2014. We
were very fortunate to secure Kate Swaffer
as the key speaker for this forum. Kate,
formally diagnosed with dementia at 49
years of age, is an internationally
renowned speaker and authority on
dementia and Alzheimer’s disease from a
perspective of how it impacts on the
individual themselves.

The forum was an opportunity for carers to
gain further insight and education about
dementia and other disability types and
also gave an opportunity for participants to
enjoy a delicious three course meal, in a
relaxed environment with people facing
similar challenges and experiences.
Where possible technology continues to be introduced to enhance existing program delivery
and/or improve efficiencies in “back-end” functions. IT systems have been upgraded at our
three dementia centres including establishing WiFi connection throughout each facility. The IT
systems were upgraded / replaced at Head Office plus Head Office systems were enhanced
to allow online/real time in-home assessments to be completed and documents printed. Staff
are now provided laptops with remote dial up access to our client data management system
(iCareHealth) to complete client assessments. CCS is currently trialling a smart-phone
application which will allow direct communication with field staff including real time scheduling
of services and management of records etc.
STAFFING & VOLUNTEERS
CCS currently employs 70 staff and is further supported by 80 volunteers all of whom make a
significant contribution to the daily lives of approximately 970 clients residing on the Central
Coast and weekly will deliver to them approximately 2,300 hours of support and assistance
across multiple programs.
During the year we welcomed Chris Golden to the Board; Diane Barry, Michelle Deren, Ilse
Bullmahn, Lyn Farmer, Ellie Doolan, Karen Fisher, Narelle Fox, Jinfeng Malcolm and Vicky
Galolo to our In Home Services program; Samantha Kiss, Claire Steinbeck & Amanda Ell to
our Dementia Social Club program; Mary Wills to our special projects; Louise Mayuga & Roslyn
Murphy to our Co-ordination Unit and Theresa Canning, Assistant to CEO & Administration
team.
Unfortunately we have farewelled our Chairperson, Annette Czerkesow who recently moved
to Rockhampton; we also farewelled Anna Staer, Veronica Candy, Sue Hammer, Celeste
Bracken and Susan Moreno.

The ongoing training & development of board members, staff (permanent & casual) and
volunteers continues to be a key focus of our organisation with in excess of 2,900 hours
recorded in our 2013/14 Training Register. In addition to mandatory training such as WHS
and manual handling, CCS also provided extensive training to staff on communicating and
working with people with dementia. We pride ourselves on the knowledge of dementia that our
staff and volunteers gain through their experience within our organisation. It is particular
pleasing to note three staff are now embarking on their final year of the inaugural Degree in
Dementia Studies from the University of Tasmania. Our more recent training sessions have
been Challenges & Perceptions of Behaviour associated with Dementia facilitated by
Alzheimer’s Australia and Understanding Dementia Massive Open Online Course (MOOC)
facilitated by the University of Tasmania. A very pleasing aspect of my role as CEO has been
to congratulate Tracey McDonough, Narelle Mitchell, and Veronica Darcy & Shayne Schuj and
present them with their Certificate 111 in Aged Care. Their training was funded under an
Australian Government initiative and in conjunction with Hunter / Central Coast TAFE. Similar
funding has also enabled fourteen staff to complete the Assist with Medication Competency
CHCCS305C.
I continue to marvel at the generosity and resilience of our volunteers. This hardy group of
people undertake all manner of activities and are a wonderful asset to our organisation. They
also embrace training opportunities offered with several recently moving into paid employment
within the sector. No doubt their job applications were strengthened by the support and
guidance provided whilst undertaking their volunteering roles with CCS. At the sake of
repeating comments made in a previous report “to our wonderful volunteers who assist so
willingly, I again extend a sincere THANK YOU; without you we could not function as effectively
and efficiently as we do.”
The past twelve months has seen senior staff play a pivotal role in the expansion of the
Community Services & Health Industry Skills Council – Workforce Innovation Network (WIN)
program on the Central Coast. The receipt of a WIN –Reform Ready Review Tailored Grant
allowed CCS, with the assistance of external consultants, to develop strategic and marketing
plans to launch Blue Sky Home Solutions as well revise our Work Health & Safety Operation’s
manual. Recognition of our efforts have been on multiple levels which included our CEO being
an invited guest at the national launch of the WIN program in Melbourne in Oct 2013. The
launch featured a video depicting Blue Sky Home Solutions as a case study. This also led to
our CEO being invited to present a paper on Collaboration & Partnering at a WIN conference
in Perth in February 2014.
CCS recognises the importance of maintaining close connections with other community
providers and maintains this philosophy through staff participation in various networking groups

During the year under review, CCS provided 1,820 direct student placement hours to 35
students from Gosford & Wyong Campus of TAFE; 6 students from Central Coast Community
College; 1 undergraduate student from each of the Universities of Sydney & New England and
3 undergraduate students from Macquarie University.

The partnership between CCS and Community Transport Central Coast continue to thrive
enabling us to manage the transportation requirements of our three dementia social clubs as
well as provision of assisted transport for clients participating in our Memory Lane Restaurants
program and general Social Outings program.
A similar partnering arrangement with Meals on Wheels Central Coast Ltd has led to the further
expansion of the Memory Lane Restaurant / Cafés programmes.
By continuing to adopt a collaborative approach and maintaining networks, CCS has now
engaged Vietnam Veterans Keith Payne VC Hostel and Woy Woy Community Aged Care Ltd
to supply meals to two of our dementia centres.
It is by developing and maintaining partnerships and collaborations that CCS is able to continue
to provide innovative, client directed excellence in the services we deliver.
My continuing thanks and appreciation to the Board and my leadership team for your support
and counsel and to all staff and volunteers for your ongoing commitment and dedication in
providing vital services to people within our community whose health and/or general well-being
jeopardises their ability to maintain the independent lifestyle of their choosing. Collectively I

SERVICE DESCRIPTIONS
CENTRE BASED SERVICES
Dementia Social Clubs
CCS conducts three Dementia Social Clubs on the Central Coast:



Geoff Wright Cottage; Henry Wheeler Place, Gosford
Yaringaa Centre; Cnr. of McMasters Rd & Ocean Beach Rd, Woy Woy
Beryl’s Place; Cnr. of Dan Close & Beryl Street, Gorokan
The centres operate Monday to Friday with GWC also operating alternate Saturdays and
Yaringaa Centre also shortly to open on alternate Saturdays.
The centres provide social day care for persons with dementia with activities well planned and
co-ordinated to ensure these activities do not overwhelm individual members. All centres are
wheelchair accessible and assistance with personal care & hygiene can be provided. Nutritious
meals & snacks are provided throughout the day and transportation from home to the centre
and return is provided.
IN-HOME SERVICES
The Social Support & Monitoring (Dementia Specific) Service
This program provides a range of services to a person with dementia living on their own. Based
on the assessed needs of a client, services monitor the health & wellbeing and provide social
support to the person with memory loss and may include:








Medication monitoring, health & safety;
Socialisation & companionship;
Meal assistance;
Personal care;
Outings, social activities;
Liaison with carers, family and doctors;
Laundry & light domestic tasks; and
Making referrals to other HACC and community care services, as required.
This service is available in both Gosford & Wyong LGAs with flexible and sensitive support 7
days a week (if required).
The Dementia ‘In-Home’ Respite Care Service
Caring for someone with dementia is sometimes described as the ‘36 hour day’ and can be
very stressful. Respite care can be provided in the client’s home or the client can be taken out
into the community for a drive, lunch, coffee or to visit a place of interest etc. Often our highly
skilled and experienced staff can further assist a carer during difficult times by offering
strategies to help them cope in their caring role.
This service is available in the Gosford and Wyong LGAs with flexible and sensitive support of
approximately 3 to 6 hours provided weekly, fortnightly, monthly or as required.

The Social Support Service
Social Support is provided by both volunteers and staff, either within a clients’ home or in the
community and includes such activities as shopping assistance, escorted transport to medical
& other appointments, companionship with a coffee & chat at a local café.
The Social Outings continue to grow in their popularity, providing a great occasion for
socialisation and friendship for our clients. They are being held fortnightly to accommodate
demand and are coordinated by our very dedicated, ever cheerful volunteer Lorraine Linnert.
Venues have included:







Road Warrior Café – Mt White
Waterview Restaurant – Berowra Waters
Shelly Beach Golf Club
Milk Factory Café - Wyong
Mooney Mooney Workers Club
Cinnamon Beans Café – Wyong
Two Birds Café – Toowoon Bay
Currently this program is only available to clients residing in the Gosford LGA.
The Domestic Assistance Service
Domestic Assistance is the single greatest support service requested by clients wishing to
remain in their own homes. The service seeks to offer cleaning of essential living areas 




Cleaning of toilets and bathrooms
Cleaning of bedroom area including light dusting
Cleaning of kitchen, cupboards and bench tops as required
Vacuuming and mopping of floors in main living areas and light dusting
Cleaning of laundry area including clothes dryer.
Some light domestic duties may also include:





Washing of essential clothes and hanging clothes out to dry
Ironing of essential clothing
Changing of bed linen
Assistance with basic food preparation
Assistance with shopping for essential grocery items and payment of bills
The Lawn & Garden Maintenance Service
This service offers regular lawn mowing (every two weeks between October – March and every
three or four weeks between Aprils – September) to 45 clients residing in the Gosford or Wyong
LGA. The service can also assist eligible clients with one-off rubbish removal / yard clean-ups.

COMMUNITY VISITORS SCHEME
This program matches volunteer visitors, one-on-one, with residents of Australian Government
funded aged care homes who are socially isolated and whose quality of life would be improved
through friendship and companionship.
Twenty five active volunteers regularly visit residents of Veronica Nursing Home, Adelene
Retirement Village, Tarragal House Nursing Home, Glenmere Nursing Home, Peninsula
Village Aged Care, and Woy Woy Aged Care & Berkeley Village Nursing Home. Each visit by
a volunteer, which might be from about 30 minutes to an hour or so, seeks to enhance selfesteem of the resident and reduce feelings of loneliness and isolation.
MEMORY LANE PROGRAM – ‘Doing Things Together’
Life should not cease when a person is diagnosed with dementia. Our Memory Lane
Program assists people living with dementia & their carers to retain their social connections &
continue a healthy lifestyle. The program consists of:
Memory Lane Restaurant

Superb dining opportunity with ambiance … music, candles, table decorations & view
of outdoor courtyard whilst a delicious three course meal is served with beverages.
Memory Lane Café

Allows people with dementia, and their carers, to meet for coffee and a chat & have
the opportunity to socialise together in a relaxing yet stimulating atmosphere.
Memory Lane Coffee Club

Dementia Carers’ Support Group; a forum for carers to have a cuppa & a chat with
others in similar situations and gain a better understanding of dementia or other forms
of memory loss.
Memory Lane Leisure Club

Opportunity for small groups to participate in social/leisure activities across the Central
Coast. Transport provided in an 8 seat van.
Memory Lane Tucker Workshops for Male Carers

Whilst the blokes are in the kitchen learning how to cook the tucker, their partners are
having a pamper day enjoying hand massages, nail painting, conversation and
laughter with cocktails and canapés. When the cooking is completed the blokes get to
show off their new culinary skills by sharing the meal they have just prepared with their
partners in a restaurant style atmosphere.
